Gamma İnterferon Salınım Testi İle Latent Tüberkülozİnfeksiyonunun Araştırılması

Özet
Amaç: Tüberkülozun ilk infeksiyonu genellikle gizli kalmakta ve vücut savunma mekanizmaları tarafından sınırlandırılmaktadır. Vücut direncinde zayıflama olursa bu infeksiyon aktif hale gelebilmektedir. Latent infeksiyon radyolojik ve bakteriyolojik olarak tanınamamaktadır. Bu infeksiyonun erken tespit edilmesi günümüzde pek çok hastalıkta kullanılan immunosupresif tedaviler öncesinde gerekmektedir. Ayrıca aile içi tüberküloz geçişinin ve küçük çocuklardaki infeksiyonun gösterilmesinde önemlidir. Gereç ve Yöntem: Çalışmaya, 20.02.2016-15.03.2017 tarihleri arasında Düzce Üniversitesi Tıp Fakültesi Hastanesi'nin servis, yoğun bakım ve polikliniklerinden tüberküloz şüphesi olan hastalar, sağlık personeli ve verem savaş dispanseri personeli olan toplam 76 kişi dahil edilmiştir. Latent infeksiyonun gösterilmesinde gama interferon salınım testi "QuantiferonTb Gold ELISA testi (QIAGEN, Almanya)" kullanılmıştır. Bulgular: Toplam 19 (% 25) kişide test pozitif olarak saptanmıştır. Pozitiflik saptanan 19 kişinin 8 (% 42)' i akciğer tüberkülozu tanısı, 4 (% 21)'ü akciğer dışı tüberküloz ön tanısı ve 7 (% 37)'si tüberkülozlu hastalarla temas etmiş olması nedeniyle çalışmaya alınmıştır. Pozitiflik saptanan kişilerin ikisi (% 10.5) akciğer tüberkülozu, biri (% 5.2) tüberküloz lenfadenit, biri (% 5.2) ise tüberküloz menenjit tanısı almıştır. Verem savaş dispanserinden çalışmaya alınan sağlık personelinin hepsinde test pozitif bulunmuştur. Son olarak, hastanemiz mikrobiyoloji laboratuvarında çalışan 19 kişiden sadece birinde test pozitif bulunmuş olup onun da ailesinde tüberküloz öyküsü olduğu öğrenilmiştir. Sonuç: Quantiferon Tb gold ELİSA testinin tüberkülozlu hastalarla sürekli temasta olan sağlık çalışanları başta olmak üzere, tüm sağlık çalışanlarında; ayrıca aktif tüberküloz infeksiyonu düşünülüp kültür sonucu çıkıncaya kadar hastanın tedavisinin erken planlanmasının gerektiği durumlarda uygulanabilecek bir yöntem olduğu düşünülmüştür.
Objective: The initial infection of tuberculosis is usually confidential and limited by body defense mechanisms. If there is weakening of the body resistance, this infection may become active. Latent infection can not be recognized as radiological and bacteriologically. Early detection of infection is necessary before immunosuppressive therapies are used in many diseases today. It is also important to demonstrated to transmission of tuberculosis from family members and presence of infections in small children. Methods: A total of 76 people were included in the study between 20.02.2016-15.03.2017 with patients with tuberculosis suspicion at Duzce University Faculty of Medicine Hospital's intensive care unit and outpatient clinics, health personel and tuberculosis dispensary personel. Gamma interferon release assay 'Quantiferon-Tb Gold ELISA test (QIAGEN, Germany)' was used to demonstrate latent infection. Results: A total of 19 (25%) subjects had a positive test. Of the 19 patients who were positive, 8 (42%) had a pulmonary tuberculosis pre-diagnosis, 4 (21%) of them had an extrapulmonary tuberculosis pre-diagnosis and 7 (37%) had contact with tuberculosis patients. Two of the patients (10.5%) were positive for lung tuberculosis, one (5.2%) were tuberculous lymphadenitis and one (5.2%) were tuberculous meningitis. All health personel who were taken to the TB dispensary were found to be positive. Finally, only one of the 19 patients have been working at microbiology laboratory of our hospital who determinate positive test results, and he had a family story of tuberculosis. Conclusion: The Quantiferon Tb gold ELISA test was thought to be a method that could be applied to all healthcare workers, especially healthcare workers who are constantly in contact with patients with tuberculosis. It is also considered appropriate to use of the patient with suspected active tuberculosis infection until culture end.
