Ağrıyı kontrol etmek ve hastalığın ilerlemesini engellemeye yönelik değişik tedavi seçenekleri mevcuttur. Ülkemizde bu tedavilerden fayda göremeyen veya bu tedavilere ulaşamayan bazı hastalar yaygın olarak romatizmal hastalıklara iyi geldiğine inanılan sülük tedavisine başvurabilmektedir. Bu yazıda 68 yaşında yaklaşık 15 yıldır osteoartrit tanısıyla medikal tedavi ve fizik tedavi gören bir olgu sunulmaktadır. Yakınmaları gerilemediği için 6 yıldır her iki dizindeki ağrılarına yönelik sülük tedavisi uygulamaya başlayan olguda; yine bir sülük tedavisi sonrası Acinetobacter spp. ye bağlı şiddetli cilt infeksiyonu gelişmiştir. In the present study, we aimed to compare the carotid IMT of ankylosing spondylitis (AS) patients with that of controls matched for age and sex. We also evaluated the risk factors for atherosclerosis related to AS. Patients and methods: Fifty-five patients with AS and 55 healthy controls were included in the study. All of the patients underwent a clinical evaluation that included the following: Bath AS Disease Activity Index (BASDAI), Bath AS Functional Index (BASFI). Bath AS Patient Global Score (BAS-G), chest expansion, and modified Schober test. The same radiologist performed IMT measurements. Results: The mean carotid IMT was 0.59 +/- 0.15 mm in patients and 0.50 +/- 0.10 nun in controls (p=0.001). Disease duration, Schober index, and chest expansion were correlated with risk of atherosclerosis in the patients with AS (r=0.301, r=-0.369, and r=0.511, respectively). Otherwise, BASDAI, BASFI, BAS-G, ESR, hsCRP and treatment seemed to have no effect on CIMT. However, there are numerous recent studies that indicate low-energy subtrochanteric femoral fractures can be associated with long-term alendronate therapy. Findings: We present a 76-year-old female patient who has taken 70 mg alendronate per week for 7 years, and who presented with pain in the left thigh. Conclusions: The patient was diagnosed as atypical femoral fracture resulting from long-term alendronate use. His symptoms began during his second season of competitive play. The distal interphalangeal (DIP) joints of the little fingers flexed 20-30, and a 10-15 valgus deformity was seen at the same joints. Pain was relieved with rest but returned immediately after playing volleyball, so plain radiographs were obtained. The flexion and valgus deformity was obvious on plain radiographs and through a clinical examination. Thus, a bilateral little-finger distal phalanx base epiphysis injury was seen. This injury is characterized by a biplanar Salter Harris physeal injury; type 5 on anteroposterior radiographs and type 2 on lateral plain radiographs. The deformity occurred as a result of competitive volleyball play. To our knowledge, this is the first reported case of a bilateral biplanar physial injury of the base of distal phalanges of the little fingers. Short-leg cast immobilization or splinting is another treatment option in such cases. To determine the clinical efficacy and antiedema effects of cohesive taping and short-leg cast immobilization in acute low-type ankle sprains of physically active patients, we performed a preliminary clinical study to assess objective evidence for edema and functional patient American Orthopaedic Foot and Ankle Society (AOFAS) scores with these alternative treatments. Methods: Fifty-nine physically active patients were included: 32 in the taping group and 27 in the short-leg cast group within a year. If a sprain was moderate (grade II) or mild (grade I), we used functional taping or short-leg cast immobilization for 10 days. We evaluated the edema and the functional scores of the injured ankle using the AOFAS Clinical Rating System on days 1, 10, and 100. Results: In each group, edema significantly decreased and AOFAS scores increased indicating that both treatment methods were effective. With the numbers available, no statistically significant difference could be detected. Conclusions: Each treatment method was effective in decreasing the edema and increasing the functional scores of the ankle. At the beginning of treatment, not only the level of edema but also the initial functional scores of the ankle and examinations are important in making decisions regarding the optimal treatment option. They have several vital functions in physiological and pathological processes in organisms. ADAMTS-9 has aggrecanolytic activity and is responsible for degradation of aggrecan mainly in articular cartilage. It is known that adiponectin is the most abundantly secreted adipokine (adipocytokines), and the characteristics of adiponectin have not been elucidated yet. It was assumed that adiponectin has anti-inflammatory effect before. However, an inflammatory feature of adiponectin was shown in researches. In our study, the effect of adiponectin on ADAMTS-9 gene expression in primary human chondrocytes was investigated. METHODS: Primary human chondrocytes were exposed to adiponectin at 1, 4, 8 and 12 mu g/ml doses for certain time period. Total RNA was isolated and reverse-transcribed by random primer after incubation. ADAMTS-9 and beta-actin genes expression levels were determined using real-time polymerase chain reaction (qRT-PCR). RESULTS: The highest upregulation of ADAMTS-9 gene expression level was found at 12 mu g/ml dose of adiponectin and 48 h incubation. CONCLUSION: Adiponectin is the key element in the maintenance of cartilage homeostasis. Similarly, the involvement of adiponectin in articular inflammatory diseases was demonstrated in detail. These findings bring adiponectin into central place in the research to develop adiponectin based new therapy methods for arthritic diseases. Together with these findings, our results suggest that adiponectin may be involved in the degradation of articular cartilage by increasing ADAMTS-9 gene expression (Tab. 1, Fig. 3, Ref. 35). Material and method: In this study histopathological evaluation of iloprost effects on knee joint synovium and cartilage are determined on young male Spraque-Dawley rats. Twenty rats divided into four groups, each consisted five rats. 0.1 ml iloprost enjected into right knees and 0.1 ml SF enjected into left knees of all rats. The groups are sacrificed randomly on first, seventh, fourteenth and twentyfirst days. Knee specimens are taken including femoral condyles, tibial platos and capsules for histopathological evaluation. Result:At the end of the histopathological evaluation, there was significant difference between the knees iloprost injected group and SF injected, control group. İn the early sacrifeced iloprost group, inflammation on joint cartilage and synovium is much more than late sacrifeced iloprost groups. İnflammation between the iloprost groups and SF control goups decreased time depentently. İn the synovial hyperplasia and synovial hypertrophy there was significant difference between the iloprost group and SF group and not changed time dependently. Conclusion:As intraarticular injected iloprost increased intraarticular inflammation we should decided not to give iloprost intraarticularly. İt is thought that systemic iloprost will have positive effects on osteoarthritic knees. © 2012 Düzce Medical Journal.Öğe The effects of diabetes on symptoms of carpal tunnel syndrome treated with mini-open surgery(2013) Işık, Cengiz; Uslu, Mustafa; İnanmaz, Mustafa; Karabekmez, Furkan Erol; Köse, KamilThis study aimed to evaluate the influence of type II diabetes mellitus (DM) on the postoperative outcomes of mini-open carpal tunnel syndrome (CTS) surgery. A total of 99 hands in 74 patients were included in the study. Of these, 36 patients (54 hands) had type II DM (Group A), and 38 patients (45 hands) had idiopathic CTS (Group B). Mini-open carpal tunnel release surgery was performed on all the hands. The night pain, weakness, paraesthesia, numbness complaints were significantly improved in both groups after surgery. However, thenar atrophy was improved significantly only in group A. Night pain, weakness, paraesthesia, numbness, and pillar pain were significantly worse in Group A than in Group B on postoperative examination. Postoperatively, Tinnel and Phalen tests were positive in 32 hands in Group A and 6 hands in Group B. Persistence of symptoms in diabetic patients was found to be more prevalent compared to non-diabetic controls after mini-open carpal tunnel release. © 2013, Acta Orthopædica Belgica.Öğe Erişkin radius distal uç kırıklarında konservatif tedavi sonuçlarının, karşı taraf el bileği ile karşılaştırılması: radyolojik ve fonksiyonel değerlendirme(2014) Uslu, Mustafa; Arıcan, Mehmet; Işık, Cengiz; Sarman, Hakan; Boyraz, İsmailAmaç: Erişkin hastalarda kapalı redüksiyon ve uzun kol alçı ile tedavi edilen distal radius uç kırıklarının radyolojik ve fonksiyonel sonuçları karşı taraf el bileği ile karşılaştırılarak değerlendirildi.Yöntemler: Ocak 2010-Aralık 2010 tarihleri arasında 77 hastaya distal radius uç kırığı için konservatif tedavi uygulandı. Kırıklar prospektif olarak incelendi ve AO ile Frykman sınıflama sistemi kullanıldı. Radyolojik ve anatomik sonuçlar Stewart skorlama kriteri ile değerlendirildi. Fonksiyonel sonuçlar kol, omuz ve el sakatlık sorgulaması (Q-DASH) ve Stewart II skorlama kriterleri değerlendirildi. Hastaların ortalama takip süresi 12 aydı.Bulgular: 40 hastanın sağ el bileğinde, 37 hastanın sol el bileğinde kırık mevcuttu. Frykman sınıflamasına göre 46 olgu tip I-II kırık, AO sınıflamasına göre 59 olguda 23,A2,1 ve 23,A2,2 kırık tespit edildi. Stewart radyolojik ve anatomik skorlama kriterine göre 77 hastanın toplam 57'sinde mükemmel, 17'sinde iyi, 3'ünde orta sonuç elde edildi. Stewart II'ye göre fonksiyonel skorlama kriterler sonuçları değerlendirildiğinde, 57 hastada mükemmel, 8 hastada iyi, 12 hastada orta sonuç alındı. Q-DASH skoru ortalama puanı 6,37 olarak bulundu. Komplikasyon oranlarımız %12,98 idi. İki hastada median sinirde hafif derecede tuzaklanma, dört hastada ulna stiloidinde kaynamama, bir hastada distal radioulnar eklemde hassasiyet, üç hastada distal radioulnar eklemde hassasiyet ve median sinirde hafif derecede tuzaklanma idi.Sonuç: Distal radius kırıkları tedavisinde kapalı redüksiyon ve alçılı tespit halen etkin ve son derece ucuz bir tedavi yöntemi olarak görülmektedirÖğe Erythropoietin accelerates fracture healing an experimental animal study(Duzce University Medical School, 2014) Türkeli, Erdinç; Uslu, Mustafa; Doğan, AliAim: The efficacy of erythropoietin (EPO) on wound healing has been shown before. There is limited data about the efficacy of EPO on fracture healing. In this study, the efficacy of EPO on closed forearm fracture healing is searched by an experimental model. Material and method: Twenty eight rats were picked randomly and divided into EPO and Control groups. On the first day, fracture was performed on the right ulna and radius of all rats. 500 U/kg daily low dose EPO was administered for each rat in EPO group and the same volume of isotonic sodium chloride solution was given intraperitoneally in the control group for five days. Seven rats from each group were sacrificed at the end of the first week; the others were sacrificed at the end of the third week. Results: At the end of the first week and third week, bone healing scores of EPO group is significantly higher than the control group according to the histological, clinic and radiologic analysis. Conclusion: EPO could have positive effects on healing of forearm fracture on rats in acute and subacute period. Low dose EPO may be a new protective agent against delayed union or nonunion in the risk population to go or not to go to surgical treatment. © 2012 Düzce Medical Journal.Öğe Erythropoietin stimulates patellar tendon healing in rats(Elsevier Science Bv, 2015) Uslu, Mustafa; Kaya, Ertuğrul; Yaykaşlı, Kürşat Oğuz; Oktay, Murat; İnanmaz, Mustafa Erkan; Işık, Cengiz; Kandiş, HayatiBackround: Erythropoietin (EPO), regulating erythropoiesis, is used to provide protective and regenerative activity in non-haematopoietic tissues. There is insufficient knowledge about the role of EPO activity in tendon healing. Therefore, we investigated the effect of EPO treatment on healing in rat patellar tendons. Methods: One hundred and twenty-six, four-month-old male Sprague-Dawley rats were randomly assigned to three experimental groups: 1, no treatment; 2, treatment with isotonic saline (NaCl) and 3, treatment with EPO. Each group was randomly subdivided into two groups for sacrifice at three (1a, 2a, 3a) or six weeks (1b, 2b, 3b). Complete incision of the left patellar tendon from the distal patellar pole was performed. We applied body casts for 20 days after the incised edges of the patellar tendon were brought together with a surgical technique. Both legs were harvested and specimens from each group underwent histological, biomechanical, and protein mRNA expression analyses. Results: There were statistically significant differences in the ultimate breaking force between the EPO group and others at both weeks three and six (p<0.05); significant differences in fibroblast proliferation, capillary vessel formation, and local inflammation were found between groups 1 a and 3a, and 2a and 3a (p<0.05). There were statistical differences between la, 3a and 2a, 3a for Col III, TGF-beta 1, and VEGF and between 1b, 3b and 2b, 3b for Col I, Col III, TGF-beta 1, and VEGF mRNA expressions. Conclusion: EPO had an additive effect with surgery on the injured tendon healing process in rats compared to the control groups biomechanically, histopathologically and with tissue protein mRNA expression.
