Primer Açık Açılı Glokomlu Olgularda Santral Kornea Kalınlığı ile Optik Sinir Başının Topografik Parametreleri Arasındaki İlişkinin Değerlendirilmesi

Özet
Amaç: Santral kornea kalınlığı (SKK) glokom için önemli prognostik faktörlerden birisidir. Primer açık açılı glokom (PAAG) olgularında SKK ile optik sinir başının konfokal tarayıcı lazer oftalmoskopi ile elde edilen topografik parametreleri arasındaki ilişkiyi değerlendirmek amacıyla çalışma gerçekleştirildi.Gereç ve Yöntem: Çalışmaya PAAG tanısı konmuş 160 olgu alındı. Olgulara tam göz muayenesi yapıldı. Konfokal tarayıcı lazer oftalmoskop ile optik sinir başının topografik parametreleri, ultrasonik pakimetri ile SKK ölçüldü. Santral kornea kalınlığı ile topografik parametreler arası olası korelasyona Sperman korelasyon analizi ile bakıldı. Yaş ve cinsiyete göre yapılan karşılaştırmalarda Mann-Whitney U testi kullanıldı. Bulgular: Olguların 90'ı kadın (%56), 70'i erkek (%44) olup yaş ortalaması 53.0315.66 idi. Ortalama kornea kalınlığı 562.3837.63 mm olup ortalama GİB 18.33.7 mmHg idi. Yaş ile SKK arasında negatif korelasyon saptandı (r-0.189; p0.043). Santral kornea kalınlığı ile çukurluk alanı (CA) (r-0.327, p0.001), CV (r-0.263, p0.001), çukurluk disk alanı oranı (CDAR) (r-0.346, p0.001), lineer çukurluk disk oranı, (r-0.358, p0.001) arasında negatif yönde korelasyon saptandı. Ortalama çukurluk derinliği arasında negatif ama anlamlı olmayan korelasyon saptandı. Santral kornea kalınlığı ile rim alanı (RA), (r0.257, p0.001), rim hacmi (RV) (r0.324, p0.001), ortalama sinir lifi kalınlığı( RNFLT) (r0.295, p0.001) ve sinir lifi çapraz kesit alanı (RNFL CSA), (r0.276, p0.001) arasında pozitif korelasyon saptandı.Sonuç: Glokomlu olgularda SKK ile optik sinir başı parametrelerinden RA, RV, ve ortalama RNFLT arasında pozitif, CA, CV, CDAR ve LCDR arasında negatif korelasyon saptanmıştır. Bu bulgular SKK'nın glokom için önemli prognostik faktörlerden biri olduğunu desteklemektedir.
Purpose: Central corneal thickness (CCT) is one of the most important prognostic factor for glaucoma. This study was performed to evaluate the relationship between CCT and topographic parameters of the optic nerve head (ONH) obtained with confocal scanning laser ophthalmoscopy in patients with primary open angle glaucoma (POAG).Material and Methods: One hundred and sixty patients with POAG were included in the study. All of the patients were underwent complete ophthalmic examination. Central corneal thickness was measured with ultrasound pachymeter. Confocal scanning laser ophthalmoscopy was performed on all patients to obtain topographic parameters of the ONH. Sperman correlation analysis was used to evaluate relationship between CCT and topographic parameters of the ONH. Comparison of the parameters in terms of age and sex were done with Mann-Whitney U test . Results: There were 90 females (%56), and 70 males (%44) with a mean age of 53.03±15.66 years. Mean CCT was 562.38±37.63 mm and mean intraocular pressure was 18.3±3.7 mmHg. There was a negative correlation between age and CCT (r-0,189; p0,043). Negative corelation was found between CCT, cup area (CA), (r-0.327, p<0.001), cup volume (CV) (r-0,263,p0.001), cup disc area ratio (CDAR) (r0.346, p<0.001), linear cup disc ratio (LCDR), (r-0.358, p<0.001). There was an insignificant negativecorrelation between CCT and mean cup depth. Positive correlation was determined between CCT, rim area (RA) (r0.257, p0.001), rim volume (RV), (r0.324, p<0.001), mean retinal nerve fiber layer thickness (RNFLT) (r0.295, p<0.001) and nerve fiber layer cross sectional area (RNFL CSA), (r0.276, p0.001).Conclusion: We found positive correlation between CCT and RA, RV, mean RNFLT and negative correlation between CCT and , CA, CV, CDAR and LCDR in patients with POAG. These data were supported that CCT is one of the important prognostic factors for glaucoma.
