Anti-Mullerian hormone (AMH) levels are high in PCOS patients and are stable throughout the menstrual cycle in healthy subjects. This study examined the influence of hormonal suppression with OC therapy on the serum AMH levels in women with PCOS and with normal menstrual cycles. Study design: Thirty women with PCOS and 15 women with normal menstrual cycles were enrolled in this prospective study. Serum was collected from the subjects during the early follicular phase of the menstrual cycle and after the sixth cycle of oral contraceptive therapy, and stored frozen until assayed. The effect of OC therapy on the serum AMH, estradiol (EA luteinizing hormone (LH), follicle-stimulating hormone (FSH), free testosterone, total testosterone, and dehydroepiandrosterone sulfate (DHEA-S) levels was studied. In addition, ovarian volume and follicle count were assessed. Results: The serum AMH levels in PCOS patients were significantly higher than in healthy women at baseline (+/- S.D.; 5.49 +/- 2.26 and 1.93 +/- 0.51 ng/ml, respectively; p = 0.001). After six cycles of OC therapy, no significant changes in the AMH levels were observed in either the PCOS patients or normally cycling women. Ultrasound showed significant reductions in ovarian volume and follicle number and size at 6 months in both groups. Conclusion: Although significant reductions were observed in ovarian volume and follicle number, 6 months of contraceptive therapy did not change the serum AMH concentration in either group. AMH may be considered a new marker in PCOS patients who are already on contraceptive treatment. (C) 2007 Elsevier Ireland Ltd. Yöntem ve gereç: Atmış rat kontrol, düşük (100 mg/kg) ve yüksek doz (200 mg/kg) L-karnitin olmak üzere 3 gruba ayrıldı. Her bir grup, parsiyel hepatektomiden sonra 24 ve 72. saat olmak üzere 2 alt gruba ayrıldı. Yedi gün intra-peritoneal L-karnitin uygulamasından sonra parsiyel (% 70) hepatektomi yapıldı. L-karnitin, parsiyel hepatektomiden sonra da ratlara verilmeye devam edildi. Tüm gruplarda cerrahi operasyondan önce, 24 ve 72 saat sonra serum IGF-1, TNF-? ve doku malondialdehid düzeyleri ölçüldü. Bulgular: Kontrol grubunda, serum TNF-? düzeyleri, rejenerasyon boyunca anlamlı derecede arttı. Düşük doz (100 mg/kg) L-karnitin, TNF-? düzeylerini azaltırken yüksek doz (200 mg/kg) artırdı. IGF-1 düzeyi, kontrol grubunda cerrahi operasyondan sonra ilk 24 saate azalırken daha sonra artmaya başladı. L-karnitin gruplarında IGF-1 düzeylerinde benzer değişimler gözlendi. Malondialdehid düzeyleri kontrol grubunda ilk 24 saatte artarken daha sonra azalmaya başladı. Benzer şekilde L-karnitin gruplarında malondialdehid düzeyleri ilk 24 saatte artarken daha sonra doza bağımlı olarak azaldı. Sonuç: Karaciğer rejenerasyonunda sitokinler, büyüme faktörleri ve metabolik yollar olmak üzere üç temel unsur bulunmaktadır. Enerji metabolizmasındaki etkilerine ilaveten, yüksek doz L-karnitinin, IGF-1, TNF-? düzeyini artırarak ve malondialdehid düzeyini de azaltarak karaciğer rejenerasyonunu hızlandırabileceğini düşünmekteyiz.Öğe Does L-carnitine increase serum TNF -alpha and IGF-1 during liver regeneration in the rat?(Tubitak Scientific & Technical Research Council Turkey, 2009) Pehlivan, Mevlüt; Coşkun, Abdurrahman; Zengin, Ahmet; Aslaner, Arif; Yavuz, TevfikAim: We sought to evaluate the effect of L-carnitine administration on the levels of IGF-1, TNF-alpha, and lipid peroxidation during hepatic regeneration for up to 72 h in an experimental partial hepatic resection rat model. Materials and methods: Sixty rats were divided into 3 groups: control and low-dose (100 mg/kg) and high-close (200 mg/kg) L-carnitine. Each group was divided into 2 sub groups (24 and 72 h after partial hepatectomy). Partial (70%) hepatectomy was performed after 7 days of intra-peritoneal administration of L-carnitine. After partial hepatectomy, L-carnitine was also administered to rats until sacrifice. Serum IGF-1, TNF-alpha, and tissue inalondialdehyde levels were determined in all groups before and at 24 and 72 h after surgery. Results: Serum TNF-alpha increased significantly in the control group during the regeneration period. The low close of L-carnitine (100 mg/kg) decreased the elevation of TNF-alpha whereas the high dose (200 mg/kg) increased it. In the control group, the IGF-1 level decreased in the first 24 h after surgery and then increased. The IGF-1 level behaved similarly in the L-carnitine groups. The inalondialdehyde level in the control group increased during the first 24 h and then decreased. Similarly in the L-carnitine groups, the malondialdehyde level increased during the first 24 h and then decreased significantly in a close-dependent manner. Conclusion: Three types of biochemical pathways are essential in liver regeneration: cytokine, growth factor, and metabolic pathways. We conclude that, in addition to its effects on energy metabolism, high-dose L-carnitine may promote liver regeneration by increasing IGF-1, TNF-alpha, and decreasing malondialdehyde.Öğe In vivo evaluation of teicoplanin- and calcium sulfate-loaded PMMA bone cement in preventing implant-related osteomyelitis in rats(Esift Srl, 2006) Tüzüner, Tolga; Şencan, İrfan; Özdemir, Davut; Alper, Murat; Duman, S.; Yavuz, Tevfik; Yıldırım, MustafaThe objective of this study was to evaluate the efficacy of teicoplanin- and calcium sulphate-loaded polymethylmethacrylate (PMMA) bone cements in preventing experimental implant-related osteomyelitis in rats. Four groups of antibiotic-loaded rods were prepared and were implanted into the lateral condylus of the rat femur after inoculation of Staphylococcus aureus. The effectiveness of these were assessed microbiologically, radiographically, and histopathologically. Radiographic evaluation revealed a significant reduction of periostal reaction and osteolysis in rats that received calcium sulphate- and teicoplanin-loaded rods. Histopathological evaluation confirmed these results. Acute infection and bone necrosis were found to be significantly lower in rats that had received calcium sulphate- and teicoplanin-loaded rods. The addition of calcium sulfate to teicoplanin-loaded PMMA bone cement appeared satisfactory as an antibiotic-carrying system for prophylaxis of experimental implant-related osteomyelitis, but further investigations are needed to reach definitive statements for clinical applications.Öğe Nasal carriage of pathogenic bacteria in medical students: effects of clinic exposure on prevalence and antibiotic susceptibility(Springer, 2007) Güçlü, Ender; Yavuz, Tevfik; Tokmak, Abdurrahman; Behçet, Mustafa; Karalı, Elif; Öztürk, Özcan; Egeli, ErolStaphylococcus aureus is a major cause of community-acquired and nosocomial infections worldwide. One important source of this pathogen for nosocomial infections is the nasal carriage of S. aureus among hospital personnel. There are only a few studies investigating the carriage of S. aureus in a community of medical students. Oral and nasal flora of 179 medical students with varying clinical exposures were determined. Oral cultures revealed no nosocomial pathogen and nasal cultures showed an increasing rate of S. aureus carriage with increasing clinical exposure. Methicillin resistance also demonstrated a tendency toward increasing with increasing clinical exposure.Öğe Phospholipase and proteinase activities in different Candida species isolated from anatomically distinct sites of healthy adults(Natl Inst Infectious Diseases, 2007) Öksüz, Şükrü; Şahin, İdris; Yıldırım, Mustafa; Gülcan, Aynur; Yavuz, Tevfik; Kaya, Demet; Koç, Ayşe NedretThe aim of the present study was to determine in vitro phospholipase and protease activities in 122 Candida spp. isolated from several anatomically distinct sites of healthy adults. C. albicans (66.4%) was the most frequently isolated Candida spp. C. glabrata (7.3%), C. tropicalis (6.3%) and C. kefyr (4.9%) were the most frequently isolated non-C. albicans Candida spp. Fifty (40.9%) of the isolates examined were phospholipase positive and 64 (52.4%) were protease positive. Forty-three (53.8%) of the C. albicans isolates tested were phospholipase producers-however, only a few strains of non-C. albicans Candida spp. behaved in the same way. Protease activity was detected in 46 (56.7%) of the C. albicans strains tested and in a few strains of non-C. albicans Candida spp. The levels of phospholipase and protease activities in commensal isolates were found to be lower than the levels of other enzyme activities previously reported in clinical Candida spp. isolates. The phospholipase activity of Candida spp. was found to be higher in oral (59.0%) and fecal (42.8%) isolates. The protease activity of Candida spp. was found to be higher in urogenital (55.1%) and skin (58.8%) isolates. We conclude that further investigations will be needed on the phospholipase and protease activity of Candida spp. in healthy subjects in order to clarify their contribution to fungal virulence.Öğe Relapsing brucella hepatitis due to Brucella abortus following doxycycline-streptomycin treatment [Doksisiklin-streptomisin tedavisi sonrasi nükseden Brucella abortus'a ba?li brusella hepatiti](2007) Özdemir, Davut; Küçükbayrak, Abdulkadir; Arabacı, Hakan; Yavuz, Tevfik; Şahin, İdris; Yıldırım, Mustafa; Şencan, İrfanWe report a case of Brucella abortus-induced brucellosis that displayed high hepatic transaminases similar to that in acute hepatitis, fourth month relapsed following doxycycline-streptomycin treatment, and developed doxycycline-related hepatotoxicity in second treatment. Brucella hepatitis may occur with various Brucella spp. and may relapse even following effective treatment.
