Sıçanlarda deneysel omurilik hasarında isotretinoinin nöral dokuda iyileşme üzerine etkileri

Özet
Retinoidler hücre büyümesini ve farklılaşmasını değiştirir. İsotretinoin, retinoik asidin aktif metabolitidir. Akne tedavisinde etkili ve remisyon süresi uzun bir ilaçtır. Şiddetli aknede, diğer tedavilere yanıtsız olan orta şiddetli aknede ve skar bırakma eğilimi olan aknede kullanılır. Akne tedavisinde bilinen etkilerinin yanı sıra doğal bağışıklık sistemini stimüle ettiği, T hücre cevabını azalttığı ve anti-inflamatuvar etki gösterdiği bilinmektedir. Çalışmamızda hücre çoğalmasını, farklılaşmasını ve apopitozu modüle eden çok sayıda genin ekspresyonunun kontrolü yoluyla omurgalı dokularının normal gelişiminde önemli bir rol oynayan A vitamini türevi olan isotretinoinin, akut omurilik yaralanması (AOY) ile oluşan hasarda nöral doku üzerinde olumlu etkisinin olup olmadığının değerlendirilmesini amaçladık. Deney ortamındaki düzenek yardımıyla sıçanlarda omurilik hasarlanması oluşturuldu. Travmayı takiben sıçanlara düşük ve yüksek doz isotretinoin gavaj şeklinde verildi. İsotretinoin tedavisinin kısa ve uzun dönemde farklı sonuçlar oluşturabileceği göze alınarak sıçanların 6'sı 2. haftanın sonunda, diğerleri ise 4. haftanın sonunda sakrifiye edilerek patolojik inceleme yapıldı. Sonuçlar omuriliği hasarlanmış ancak isotretinoin verilmeyen sıçanların sonuçları ile karşılaştırıldı. Eğik düzlem, Drummond ve Moore Kriterleri, Hemotoksilen-eozin, Luxol fast ve Cleavedcaspase 3 immunohistokimyasal boyamasının sonuçlarına göre isotretinoin AOY ile oluşan hasarın giderilmesinde etkili bulunmuştur. Tedavi süresi uzadığında ve tedavi dozu yükseldiğinde bu etkinin arttığı izlenmiştir. İsotretinoinin AOY tedavisinde faydalı olabileceği görülmüştür. Ancak daha geniş serilerde yapılmış klinik ve laboratuvar çalışmalarıyla bulguların desteklenmesi gerekmektedir. Anahtar Kelimeler: Isotretinoin, Akut omurilik yaralanması, Antiinflamatuvar
Retinoids alter cell growth and differentiation. Isotretinoin is the active metabolite of retinoic acid. It is a drug that is effective in the treatment of acne and has a long remission period. It is used in severe acne, moderate acne unresponsive to other treatments, and acne with a tendency to scarring. In addition to its known effects in the treatment of acne, it is known that it stimulates the natural immune system, reduces the T cell response and has an antiinflammatory effect. In our study, we aimed to evaluate whether isotretinoin,vitamin a derivative, which plays an important role in the normal development of vertebrate tissues by controlling the expression of many genes that modulate cell proliferation, differentiation and apoptosis, has a positive effect on neural tissue in damage caused by acute spinal cord injury (ASCİ). With the help of the apparatus in the experimental environment, spinal cord injury was created in rats. Following trauma, rats were given low and high dose isotretinoin by gavage. Considering that isotretinoin treatment may produce different results in the short and long term, 6 of the rats were sacrificed at the end of the 2nd week and the others at the end of the 4th week, and pathological examination was performed. The results were compared with those of rats with spinal cord injury but not given isotretinoin. According to the results of inclined plane, Drummond and Moore Criteria, Hemotoxylin-eosin, Luxol fast and Cleaved caspase 3 immunohistochemical staining, isotretinoin was found to be effective in removing the damage caused by ASCİ. It has been observed that this effect increases when the duration of treatment is prolonged and thetreatment dose is increased. It has been observed that isotretinoin may be beneficial in the treatment of ASCİ. However, the findings need to be supported by clinical and laboratory studies conducted in larger series.
