Abstract
Amaç: Wechsler Mental Durum Ölçeği Yeniden düzenlenmiş (WMDÖ) ve Mini Mental Durum Ölçeği (MMDÖ) skorları hafif düzeydeki demans olgularında tanıda ve takipte oldukça kullanışlıdır. Gereç ve Yöntem:Hastaların Türkiye’de kalabalık normal poliklinik şartlarında veya aile hekimi taarfından değerlendirilmesi demans tanısı için yeterli değildir. Bu çalışmada yukarıda bahsi geçen ortamlarda veya aile hekimi tarafından demans tanısı almış olgular yeniden değerlendirildi. Fonksiyonel bellek problemi olan 26 hastaya WMDÖ ve MMDÖ ile birlikte muayene edildi. Depresyon tanısı DSM-IV tanı kriterleri kullanılarak konuldu. Bulgular: Altı ay süren çalışmamızda değerlendirmeler sonrasında, anormal bellek bozukluğunun 11 hastada demans sednromu ile ilişkili olmadığını bulduk. Depresyon bu 11 hastada görülen en yaygın problemdi ve hastaların %78’inde saptandı. On beş demans hastasının ise 3’ünde (%20) depresyon ve 3’ünde (%20) ajitasyon mevcuttu. Sonuç: Normal poliklinik şartlarında muayene veya aile hekimi tarafından değerlendirilmesi demans tanısı için yeterli değildir. Bellek problemi olan halkın değerlendirlmesinde ayrıntılı nöropsikiyatrik değerlendirmeye yardımcı olarak WMDÖ’nün kullanılabilir bir test olduğu sonucuna vardık.
Aim : Wechsler Memory Scale-Revised [WMS-R] and the scores from the Mini-Mental State Examination [MMSE]) are highly useful in the diagnosis and follow-up of mild dementia. Material and method: The evaluation of the dementia patients in normal outpatient clinics which are crowded in Turkey or evaluation by family doctors are not enough for the diagnosis. In this study, the patients who had diagnosis of dementia in the conditions above mentioned were reexamined. We examined the performance on the WMS-R and MMSE of 26 patients with a functional mental problem. The presence of depression was evaluated by using DSM-IV criteria. Results: After examinations during 6 months, we have found that abnormal memory impairment was not related to the dementia syndrome in 11 patients. Depression (78%) was the most common problem in these 11 patients. There was depression in 3 (20%) and agitation in 3 (20%) of 15 dementia patients. Discussion: Normal neurology outpatient clinics or family doctor examinations are not enough for the diagnosis of dementia. We conclude that in addition to the detailed neuropsychiatric evaluation, the WMS-R is a useful test for the population with memory problems.
