Özet
Migren, bulantı ve kusmanın eşlik ettiği, tekrarlayan orta veya şiddetli ataklarla kendini gösteren, 4-72 saat süren bir baş ağrısı türüdür. Hormonal, çevresel ve genetik faktörler migren duyarlılığında önemli rol oynamaktadırlar. Karmaşık bir durum olan migren fizyopatolojisi hala tam olarak anlaşılamamıştır. Ancak yapılan çalışmalarda vazodilatatör bir takım ajanların migren oluşumunda etkili olduğu düşünülmektedir. Bu çalışmada vazodilatatör etkiye sahip bir nöropeptid olan kolesistokininin migrene etkisi araştırılmıştır. Kesitsel ve tanımlayıcı tipte tasarlananmış olan bu araştırmanın amacı, aurasız migren tanısı almış hastalar ile migren tanısı almamış bireylerde plazma kolesistokinin düzeyleri arasında bir farklılık olup olmadığının incelenmesidir. Düzce Üniversitesi Uygulama ve Araştırma Merkezi Nöroloji Polikliniği tarafından Aurasız Migren tanısı almış 36 hasta migrenli grubu, migren tanısı olmayan ve hastanenin diğer polikliniklerine başvurmuş 37 kişi de kontrol grubunu oluşturdu. Veri toplama aracı olarak araştırmacının düzenlemiş olduğu Hasta Bilgi Formu kullanıldı ve çalışmaya katılan kişilerden kan örnekleri alınarak laboratuar ortamında çalışıldı. Elde edilen veriler SPSS 22.0 programı kullanılarak bilgisayar ortamında bir istatistik uzmanından danışmanlık alınarak değerlendirildi. Katılımcıların sosyo-demografik özellikleri, tıbbi öyküleri, vital bulguları ve migren hastalığına ilişkin bilgileri tanımlayıcı istatistikler (yüzde, sayı, ortalama) ile değerlendirildi. Migren grubu ve kontrol grubunun karşılaştırmalı analizlerinde alt gruplar arasındaki karşılaştırmalarda x² testi, ortalamalar arasındaki karşılaştırmalarda non-parametrik test olarak kullanılması önerilen Mann Whitney U ve Kruskal Wallis testi kullanıldı. İstatistiklerde p anlamlılık için güven aralığı p?0.05 olarak kabul edildi. Çalışmaya katılan gruplar içerisinde migrenli grubun yaş ortalaması 35,53 ± 6,83, kontrol grubunun yaş ortalaması 34,99 ± 7,79, toplam grup yaş ortalaması 35,23 ± 7,29'dur. Migrenli grup ve kontrol grubunun kolesistokinin (CCK) değerleri karşılaştırıldığında, migrenli grubun CCK değeri 1,83 ± 0,60 olduğu görülürken, kontrol grubunun CCK değeri 1,73 ± 0,49 olarak bulunmuştur.
Migraine is a type of a headache that lasts 4-72 hours, accompanied by nausea and vomiting, manifested by recurrent moderate or severe attacks. Hormonal, environmental and genetic factors play an important role in migraine sensitivity. The complicated condition of migraine pathophysiology is still not completely understood. However, in studies it is considered that some vasodilator agents are effective in migraine formation. Therefore, we researched the migraine effect of cholecystokinin, a neuropeptide with vasodilator effect. The aim of this cross-sectional and descriptive study is to examine whether there is a difference between plasma cholecystokinin levels in patients with migraine without aura and in migraine-free patients. Thirty-six migraine patients who were diagnosed as Migraine without aura by Düzce University Application and Research Center Neurology Policlinic were enrolled in the study, addingly, thirty-seven patients who were not migraine patients and referred to other outpatient clinics of the hospital participated in the study as a control group.The patient information form was used as the data collection tool and the blood samples were taken from the participants and examined in the laboratory environment. The obtained data were evaluated by consulting with a statistical expert on a computer using SPSS 22.0 program. Participants' sociodemographic characteristics, medical history, vital signs, and knowledge of migraine were assessed by descriptive statistics (percent, number, mean). In the comparative analyzes of the migraine group and the control group, the X² test was used in the comparison between the subgroups, and the Mann Whitney U and Kruskal Wallis test, which was recommended as a non-parametric test, were used in the comparison between the averages. In the statistics p confidence interval for p significance was accepted as p?0.05. The mean age of the migraine group was 35,53 ± 6,83, the mean age of the control group was 34,99 ± 7,79, and the total group mean age was 35,23 ± 7,29.
