dc.description.abstractBu çalışmada artroskopik girişimlerde VİMA (Volatil induction and maintanence anesthesia) ve TİVA (Total intravenöz anestezi) indüksiyonu sonrası LMA uygulamasının solunum fonksiyonlarına etkisini karşılaştırmayı amaçladık.Gereç ve Yöntem: Çalışmamız Düzce Üniversitesi Tıp Fakültesi Hastanesi Ameliyathanesi ve Göğüs Hastalıkları polikliniğinde gerçekleştirildi.Bilgilendirilmiş hasta onamları alındıktan sonra genel anestezi ile operasyonu planlanan ASA 1-2, 18-60 yaş arası artroskopi operasyonu olacak 50 hasta çalışmaya dahil edildi. Operasyon odasında hastaların elektrokardiyografi ( EKG ), sistolik kan basıncı ( SKB ), diastolik kan basıncı (DKB), ortalama kan basıncı (OKB), kalp atım hızı (KAH), periferik oksijen satürasyonu (SPO2) monitorize edildi.Çalışmaya dahil edilen tüm hastalara Göğüs Hastalıkları polikliniğinde preoperatif, postoperatif 2.saat ve postoperatif 24.saat solunum fonksiyon testi yapıldı. Test 3 sefer tekrar edilerek en iyi FVC ve FEV1 değerleri seçildi.Bulgular: Postoperatif periyottaki FEV1, FVC, FEF %25-75 değerleri preoperatif ve taburculuktaki duruma göre anlamlı düzeyde düşük çıkmış ancak preoperatif ve taburculuktaki FEV1, FVC, FEF %25-75 değerleri arasında anlamlı fark bulunamamıştır. Bunların yanısıra TİVA grubundaki FEV1 ve FVC ortalaması VİMA grubuna göre her 3 ölçüm periyodunda da anlamlı düzeyde düşük bulunmuştur.FEV1/ FVC bakımından yapılan değerlendirme sonucunda anestezi grupları arasında anlamlı farka rastlanmamıştır. Ayrıca preoperatif, postoperatif ve taburculuk öncesi ölçüm periyotları arasında da anlamlı farka rastlanmamıştır.Tartışma: Sonuç olarak çalışmamızda genel anestezi sonrasında her iki grupta da FVC, FEV1, FEF %25-75 gibi respiratuar parametrelerde azalma saptadık. dc.description.abstractPurpose: In this study, our purpose was to compare the effects of LMA appliance after VIMA or TIVA induction on respiratory functions in arthroscopic procedures.Material-method: The study was performed in operating room and polyclinic on pulmonary diseases in Medical Faculty Hospital of the University of Duzce. Fifty patients, who were undergoing arthroscopic surgery under general anesthesia between age 18-60 and classified as ASA 1-2 were included after their informed consent was taken. In the operating room electrocardiography (ECG), systolic blood pressure (SBP), diastolic blood pressure (DBP), mean blood pressure (MBP), heart rate (HR), peripheric oxygen saturation (SPO2) were monitorised.Respiratory function test was performed to all patients included, preoperatively, on 2nd hour postoperatively and on 24th hour postoperatively in the sitting position, after defining how the test works. The test was performed 3 times each and the highest FVC and FEV1 values were selected.Results: FEV1, FVC, FEF%25-75 values postoperatively were significant lower than the values preoperatively and by discharge, but between FEV1, FVC, FEF%25-75 values preoperatively and by discharge were no significant difference. Additionally, FEV1 and FVC values in group VIMA were significantly lower than in group TIVA on every 3 test periods.FEV1/FVC values were not significantly different between two anesthesia groups. In addition to that, there was no significantly difference between test periods preoperatively, postoperatively and by discharge.Conclusion: In conclusion, we determined by each two anesthesia groups that respiratory parameters like FVC, FEV1, FEF %25-75 were decreased. By each two anesthesia groups were lung functions decreased and FEV1/FVC values were remained.
