Traction alopecia is a traumatic hair loss as a result of excessive pulling of the hair on the margins or nonmarginal area of the scalp due to hairstyles. Peripilar keratin casts is a cylindrical structure that encircles the hair shaft placed at the periphery of the alopecia plaque. It is derived from the inner and/or outer hair root sheath cells and it develops because of persistent hair traction. The most common misdiagnosed is louse eggs or nits. Other diagnosis that must be considered include squamous disease of the scalp such as psoriasis, pityriasis simplex and seborrheic dermatitis. This structure is a useful dermoscopic finding for physician in both diagnosis and follow-up of traction alopecia. Here, we describe a thirty-four-year-old women with traction alopecia whose diagnosis is clarified by the presence of peripilar keratin cast. © 2015 by Türkiye Klinikleri.en_US
