Özet
Bronşektazinin patogenezini iki temel mekanizma oluşturur. Birinci mekanizma bronşların obstr uuml;ksiyonu veya anormal dilatasyonu, ikinci mekanizma ise kronik enfeksiyonlardır. Bu bildiride hiatal herninin bronşa dıştan basıyla oluşturduğu obstruksiyon nedeniyle oluşmuş bronşektazi olgusu sunulmuştur. 77 yaşında bayan hasta, 3 g uuml;n ouml;nce bir kez olmuş balgamla karışık kan t uuml;k uuml;rme şikayetiyle polikliniğimize başvurdu. Ouml;zge ccedil;miş ve soyge ccedil;mişinde ouml;nemli bir ouml;zellik mevcut değildi. Solunum sistemi muayenesi normal olarak bulundu. Ccedil;ekilen posteroanterior akciğer grafisinde kalple s uuml;perpose hava sıvı seviyesi izlendi. Ccedil;ekilen Toraks Bilgisayarlı Tomografi (BT) rsquo;sinde hiatal herni ve lingulada bronşektazi saptandı. Sonu ccedil; olarak bronşektazi etyolojisinde yer alan dıştan basıya neden olan t uuml;m ouml;r ve lenfadenopati dışında hiatal herninin de dıştan bası ile bronşektaziye neden olabileceği d uuml;ş uuml;n uuml;lmektedir.
The pathogenesis of the bronchiectasis is based on two basic mechanisms. Airflow obstruction or abnormal dilatationof the bronchi is the first mechanism. The second mechanism is chronic infections of the airways. This report is relatedwith bronchiectasis occurring obstruction of bronchi due to external compression of hiatal hernia. 77 years old femalepatient admitted to our clinic with the complaint of hemoptysis. No other abnormal history was present and physicalexamination was normal. Posteroanterior chest radiograph revealed "air fluid level" behind heart. Thorax ComputerizedTomography showed a hiatal hernia and lingual segment bronchiectasis. As a result, we present a case with bronchiectasisdue to obstruction of bronchi due to external compression of hiatal hernia other than tumors or lymphadenopathies.
