The compliance to hormone replacement therapy is decreased all over the world after the release of the findings from randomized, prospective, controlled trials about the cardiovascular effects of hormone therapy. The interest around dietary supplement products specifically targeting women in menopause have increased. The raise in their popularity makes it necessary to examine the efficacy and tolerability of these compounds. This review analyzes the prospective, randomized, controlled trials investigating the effects of phytoestrogens on hot flashes, cardiovascular system, breast, endometrium, and cognitive functions. The overall efficacy of these compounds in the climacteric symptoms is, at best, about half of that obtained with standard hormone replacement therapy. Only soy nutrition from the beginning of life onwards, particularly during puberty, may have mammary cancer preventing effects. The meta-analysis failed to show their cardioprotective effects. Long-term treatment with soy phytoestrogens was associated with an increased occurrence of endometrial hyperplasia. As a result the efficacy and safety of phytoestrogen treatment, as an alternative to hormone replacement therapy in preventing menopausal symptoms, is not proven yet.
