Atriyal fibrilasyon ablasyonu yapılan hastların yaşam kalitesi ve etkileyen faktörlerin incelenmesi

Özet
Bu çalışma, atriyal fibrilasyon(AF) ablasyonu yapılan hastaların yaşam kalitesi ve etkileyen faktörlerin incelenmesi amacıyla yapılmıştır. Tanımlayıcı tipte yapılan araştırmaya, Hacettepe Üniversitesi Erişkin Hastanesi Kardiyoloji Anabilim Dalı' nda takip edilen, AF ablasyon yapılan, işlemden en az üç ay geçmiş, kontrol amaçlı polikliniğe başvuran veya herhangi bir nedenle hastanede yatan, 18 yaş ve üzerinde 72 hasta dahil edilmiştir. Veriler, katılımcıların sosyodemografik özelliklerini ve sağlık durum alışkanlıklarını değerlendirmek amacıyla oluşturulan, yapılandırılmış soru formu ve Atriyal Fibrilasyonun Yaşam Kalitesi Üzerine Etkisi Anketi(AFEQT) kullanılarak 01/12/2019 ve 01/05/2020 tarihleri arasında toplanmıştır. Verilerin değerlendirilmesinde SPSS V23 kullanılmıştır. Analiz sonuçları nicel veriler için ortalama±standart sapma ve ortanca(minimum - maksimum) şeklinde, kategorik veriler için frekans ve yüzde olarak sunulmuştur. Katılımcıların AF' ye bağlı en sık yaşadıkları şikayetlerin çarpıntı(%93.1), halsizlik(%52.8), yorgunluk(%45.8) ve nefes darlığı(%38.9) olduğu, katılımcıların %51.4' ünün genel sağlık durumlarının "iyi" olduğu, geçtiğimiz yıla göre sağlık durumunu kıyasladıklarında ise %36.1' inin "aynı" %8.3' ünün ise "daha kötü" olduğu görülmüştür. Ölçek puanları incelendiğinde belirtiler alt boyutu puan ortalaması 72.6±27.3, günlük etkiler alt boyutu puan ortalaması 58±32.9, tedavi kaygısı alt boyutu 73.1±27.3, tedavi memnuniyeti alt boyut puan ortalaması 65.7±25 ve ölçek toplam puan ortalaması 66.2±26.2 ile katılımcıların yaşam kalitesi yüksek bulunmuştur. Yaşam kalitesi ve etkileyen etmenlere yönelik bütüncül bakımın sürdürülmesi için periyodik uzun dönem takipler ile bireylerin yaşam kalitesinin ölçeklerle değerlendirilmesi, kılavuzların oluşturulması, spesifik değerlendirme, takip, eğitim planları oluşturulması önerilmektedir.
This study was conducted to investigate the quality of life and the factors affecting the quality of life of patients who underwent atrial fibrillation(AF) ablation. In this descriptive study, 72 patients who were 18 years old and above, were followed in Hacettepe University Adult Hospital Cardiology Department, had undergone AF ablation, had at least three months passed after the procedure, were hospitalized for control or for any reason were included. The data were collected between 01/12/2019 and 01/05/2020 using a structured questionnaire which was created to evaluate the sociodemographic characteristics and health habits of the participantsand the Effect of Atrial Fibrillation on Quality of Life(AFEQT) questionnaire. SPSS V23 was used to evaluate the data. Analysis results were presented as mean ± standard deviation and median (minimum-maximum) for quantitative data, and as frequency and percentage for categorical data. The most common complaints of the participants due to AF were palpitation (93.1%), weakness(52.8%), fatigue (45.8%) and shortness of breath (38.9%). It was observed that the general health status of 51.4% of the participants was "good", and when compared to the previous year, 36.1% of them were "the same" and 8.3% were "worse". When the scale scores were examined, the mean score for the symptoms sub-dimension was 72.6±27.3, the mean for the daily effects dimension was 58±32.9, the treatment anxiety sub-dimension was 73.1±27.3, the mean for the treatment satisfaction sub-dimension was 65.7±25, and the mean score for the total scale was 66.2±26.2, and the participants' quality of life was found to be high. In order to maintain holistic care for quality of life and affecting factors, it is recommended to evaluate the quality of life of individuals with scales with periodic long-term follow-ups, to create guidelines, to create specific evaluation, follow-up and training plans.
