Parkinson hastalarında izokinetik egzersiz programının denge, bradikinezi ve yaşam kalitesi üzerindeki etkisi

Özet
Amaç: Bu çalışmanın amacı, Parkinson hastalarında izokinetik egzersiz programının denge, bradikinezi ve yaşam kalitesi üzerindeki etkisini değerlendirmektir. Gereç ve Yöntem: Bu çalışmaya, 2022–2024 yılları arasında Düzce Üniversitesi Tıp Fakültesi Fiziksel Tıp ve Rehabilitasyon Anabilim Dalı'na başvuran, İngiltere Parkinson Hastalığı Beyin Bankası kriterlerine göre idiyopatik Parkinson hastalığı tanısı konulmuş ve Hoehn–Yahr evresi ≤3 olan 50 hasta dahil edilmiştir. Hastalar konvansiyonel rehabilitasyon ve konvansiyonel rehabilitasyona ek izokinetik egzersiz uygulanan grup olmak üzere iki gruba ayrılmıştır. Tüm hastalara haftada 5 gün konvansiyonel rehabilitasyon uygulanmış; izokinetik egzersiz grubuna ek olarak haftada 3 gün 60°/sn ve 180°/sn açısal hızlarda diz fleksör ve ekstansör kaslara yönelik izokinetik güçlendirme egzersizi uygulanmıştır. Tüm hastalar tedavi öncesinde ve 6. hafta sonunda klinik ve izokinetik ölçümler yapılarak değerlendirilmiştir. Bulgular: Her iki grupta tedavi sonrası klinik ölçeklerde anlamlı iyileşmeler saptanmıştır (tümü p<0,001). Gruplar arası değişim karşılaştırmalarında genel olarak anlamlı fark bulunmamıştır (p>0,05). Kovaryantlarla ayarlanmış analizlerde yalnızca UPDRS-III için izokinetik egzersiz lehine anlamlı grup etkisi görülmüştür (β = -3,14; p = 0,007). İzokinetik değerlendirmelerde her iki grupta artışlar izlenmiş; sağ diz ekstansiyonuna ait bazı parametrelerde izokinetik egzersiz grubunda daha fazla artış saptanmıştır (p<0,05). Diz ekstansiyon kuvvetindeki artışın yürüme hızı ve tek ayak durma süresindeki artış ile pozitif, Parkinson Hastalığı Yaşam Kalitesi Anketi (PDQ-39) değişimi ile negatif yönde korelasyon gösterdiği belirlenmiştir (p<0,05). Sonuç: Her iki rehabilitasyon yaklaşımı fonksiyonel durum ve yaşam kalitesini iyileştirmiştir. İzokinetik egzersiz bazı motor parametrelerde ek katkı sağlamakla birlikte klinik açıdan belirgin bir üstünlük göstermemiştir. Daha geniş örneklemli ve uzun süreli çalışmalara ihtiyaç vardır. Objective: The aim of this study was to evaluate the effects of an isokinetic exercise program on balance, bradykinesia, and quality of life in patients with Parkinson's disease. Materials and Methods: Fifty patients who were admitted to the Department of Physical Medicine and Rehabilitation at Düzce University Faculty of Medicine between 2022 and 2024, diagnosed with idiopathic Parkinson's disease according to the United Kingdom Parkinson's Disease Society Brain Bank criteria, and classified as Hoehn–Yahr stage ≤3 were included in the study. The patients were divided into two groups: a conventional rehabilitation group and a group receiving isokinetic exercise in addition to conventional rehabilitation. All patients underwent conventional rehabilitation five days per week. The isokinetic exercise group additionally performed isokinetic strengthening exercises targeting the knee flexor and extensor muscles three days per week at angular velocities of 60°/s and 180°/s. All patients were evaluated before treatment and at the end of the 6th week using clinical and isokinetic measurements. Results: Significant improvements were observed in clinical scales in both groups after treatment (all p<0.001). In between-group comparisons of changes, no significant differences were found overall (p>0.05). In analyses adjusted for covariates, a significant group effect in favor of the isokinetic exercise group was observed only for UPDRS-III (β = −3.14; p = 0.007). In isokinetic assessments, increases were observed in both groups; some right knee extension parameters showed greater improvement in the isokinetic exercise group (p<0.05). The increase in knee extensor strength was positively correlated with improvements in gait speed and single-leg stance time, and negatively correlated with changes in the Parkinson's Disease Questionnaire (PDQ-39) scores (p<0.05). Conclusion: Both rehabilitation approaches improved functional status and quality of life. Although isokinetic exercise provided additional benefits in certain motor parameters, it did not demonstrate a clinically significant superiority. Larger-scale and long-term studies are needed.
