dc.description.abstractGiriş: OUAS ciddiyetini gösteren AHI ve gece boyu saptananen düşük oksijen satürasyonu ile insülin direnci arasında net bir ilişki bulunduğu gösterilmiştir. Uyku ile ilişkili hipokseminin şiddeti ile insülin direnci arasında bulunan anlamlı korelasyon, uyku apnesinin metabolik etkilerinin apne ve hipopne ile ortaya çıkan oksijen desatürasyonu ile ilişkili olduğunu desteklemektedir. Bu çalışma da bizde OUAS ile obeziteden bağımsız olarak insülin direnci arasında ki ilişkiyi araştırdık.Materyal-Metod: Göğüs Hastalıkları polikliniğine Ekim 2006- Mayıs 2008 tarihleri arasında başvuran obez 40 hasta polisomnografiye bağlanarak OUAS tanısı alan grupla OUAS tanısı almayan grup arasında insülin direnci bakıldı.Bulgular: OUAS'u olan grupta 13 hastada insülin direnci saptanırken (%65), OUAS'u olmayan bireylerden oluşan kontrol grubunda 11 kişide insülin direnci saptandı (%55). Aradaki fark istatiksel anlamlı değildi. Ancak her iki gruptaki bireylerin hepsi obez olmasına rağmen OUAS'u olan grupta OUAS'u olmayan gruba göre daha yüksek oranda insülin direnci saptandı.Sonuçlar: Bu çalışmada her ne kadar biz OUAS ile insülin direnci arasında direkt bir ilişki gösterememiş olsakda bu grup hastadaki insülin direncinin yüksekliği olası ortak risk faktörleri ve/veya sonuçları olabilir. OUAS hastalarında insülin direnci gelişiminin mültifaktöriyel olduğunu ve obezitenin major belirteç olduğunu söyleyebiliriz. Yinede uzun süreli izlenen daha büyük hasta gruplarının yer aldığı karşılaştırmalı, prospektif çalışmalara gereksinim vardır.en_US
dc.description.abstract8.İntroduction: The relationship between insulin rezistance and AHİ that indicates OSAS seriousness and also the relationship betweeb insülin resistance and low O2 saturation are showed. The meaningful corelation between the hipoksemi severity related with sleep and the insulin resistance, supports that there is relationship between the metabolic effects of sleeping apne and O2 dasaturation because of apne or hipoapne. İn this study, we search the relationship between obstructive sleeping apne sendrom (OSAS) and the insulin resistance independent from obesity.Material-method: 40 Pations who apply chest policlinics between the dates july 2007-may 2008 are tied to polysomnografi 13 patients of 40 get diagnosis as OSAS and they form the first group. The rest who dont get diagnosis as OSAS form the second group and all of them are searched for insülin resistance.Findings: All of the frist group,involver 13 patients who have OSAS, have insulin resistance but insulin resistance are found out at only 11 patients of the second group, involve who dont have OSAS. These results are not meaningful for the two groups. But, although all the members of two groups are obes, the first group have more high insulin resistance than the second.Results: At this study, we cant indicate the direct relationship between OSAS and insülin resistance but according to the other studies we can say that insülin resistance progress is multifactorial and obesity is the major sign Aspecially after the decrease of insülin resistance at the third day of CPAP cure, we think that the obesity is nat sole factor. We need comparative, prospective studies which have big groups of patients who are known for a long time.en_US
