Kronik Kalp Yetersizliği Olan Hasta Grubunda İlaç Uyumu Yaşam Kalitesini Etkiliyor Mu?

Özet
Amaç: Çalışmamız kalp yetersizliği olan hastaların tedaviyeuyumları ile yaşam kalitesi arasındaki ilişkiyi incelemekamacıyla tanımlayıcı olarak yapılmıştır.Materyal-Metot: Çalışmamızın evrenini, GaziantepÜniversitesi Şahinbey Araştırma ve Uygulama Hastanesive SANKO Üniversitesi Hastanelerinde Koroner YoğunBakım ve Kardiyoloji Kliniği'nde Kasım 2017-Temmuz 2018tarihleri arasında Kronik Kalp Yetersizliği nedeniyle tedavigören hastalar, örneklemini ise araştırmamızın kriterlerineuyum sağlayan 100 hasta birey oluşturmuştur.Bulgular: MacNew kalp hastalığına özgü sağlıkla ilişkiliyaşam kalitesi anketi ve ilaç kullanmaya ilişkin sağlık inançölçeği skorları arasında istatistiksel olarak anlamlı düzeydebir ilişki saptanmamıştır. Erkek cinsiyetin yaşam kalitesiskorları kadınlara göre daha yüksek iken, ilaç uyumu ilecinsiyet arasında herhangi bir ilişki bulunamamıştır. Eğitimdurumu ile ilaç uyum ölçeği puanları arasında da ilişkibulunamamışken, eğitim düzeyinin düşük olmasının yaşamkalitesini düşürdüğü görülmüştür. BKİ ile ilaç uyum ölçeğiskorları arasında ilişki bulanamamışken, obezlerin fiziksel altboyutta yaşam kalitelerinin daha düşük olduğu görülmüştür.Sonuç: Bu çalışmanın sonunda ilaç uyumu ile yaşam kalitesiarasında anlamlı bir ilişki bulunamamış, kalp yetersizliğiolan kadın hastaların yaşam kalitesi ve uyum düzeyleriniyükseltmek üzere girişimlerin planlanması ve ileri düzeykalp yetersizliği olan hastaların tedavi uyumlarının başkabireylere bağımlı olması nedeniyle evde bakım ve hemşirelikhizmetlerinin yaygınlaştırılması önerilmiştir.
Objective: Our study was conducted descriptively to investigate the correlation between the treatment compliances of patients with heart failure and the quality of life. Material-Method: The universe of our study consisted of patients who were treated for chronic heart failure in Coronary Intensive Care and Cardiology Clinic of Gaziantep University Şahinbey Research and Practice Hospital and SANKO University Hospitals between November 2017 and July 2018. Results: There was no statistically significant correlation between the MacNew health-related quality of life questionnaire and the drug use health beliefs scale scores. The quality of life scores of males were higher than that of females, while there was no correlation between drug compliance and gender. While no correlation was found between educational status and the drug compliance scale scores, it was observed that low level of education decreased the quality of life. While no correlation was found between body mass index (BMI) and the drug compliance scale scores, it was observed that the obese patients had a lower quality of life in the physical sub-dimension. Conclusions: At the end of this study, no significant correlation was found between drug compliance and quality of life. It has been suggested to plan interventions to increase the quality of life and drug compliance of female patients with heart failure, and to extend home care and nursing services since the treatment compliance of patients with advanced heart failure is dependent on other individuals.
