Özet
Araştırma, hemşirelerin olay bildirim sistemlerini kullanımının ve etkileyen faktörlerin incelenmesi amacıyla tanımlayıcı olarak yapılmıştır. Araştırmanın evrenini, bir kamu hastanesinde çalışan 348 hemşire oluşturdu. Araştırma verileri, Nisan 2022-Haziran 2022 tarihleri arasında, araştırmaya katılmayı kabul eden 264 hemşire ile "Demografik Bilgi Formu", "Olay Bildirim Farkındalık ve Kullanım Bilgi Formu", "Olay Bildirimi Tutum Ölçeği (OBTÖ)", "Güvenlik Tutumu Ölçeği (GTÖ)", "Olay Bildirim Engelleri Ölçeği (OBEÖ)" kullanılarak yüz yüze görüşme tekniği ile toplanmıştır. Araştırma veri analizinde tanımlayıcı istatistikler, Pearson korelasyon katsayısı, bağımsız t testleri, tek yönlü varyans analizi (One-Way ANOVA) kullanılmıştır. Analiz sonuçlarına göre, hemşirelerin %44,7'nin herhangi bir olay bildirim eğitimi almadığı, %39'unun meslekte rapor edilmesi gereken hasta güvenliğine yönelik bir olaya tanık veya dâhil olduğu bununla birlikte sadece %37,9'nun hasta güvenliğine yönelik olay bildiriminde bulunduğu, hemşirelerin %81,8'nin olay bildirim sistemi farkındalığı ve %55,3'ünün olay bildirim kullanım bilgisi olduğu belirlenmiştir. Araştırma kapsamına alınan hemşirelerin OBTÖ puan ortalaması 2,24±0,34 olarak bulunmuş ve olay bildirimine ilişin tutumlarının kısmen olumsuz olduğu, GTÖ puan ortalaması 3,44±0,50 olarak bulunmuş ve güvenlik tutumlarının olumlu olduğu, OBEÖ puan ortalaması 2,70±0,72 olarak bulunmuş ve olay bildirim engel algıları kısmen yüksek olduğu belirlenmiştir. Hemşirelerin olay bildirme durumları ile olay bildirim tutumları, güvenlik tutumları, olay bildirim engelleri arasında istatistiksel olarak anlamlı derecede bir fark bulunmuştur (p<0,05). Araştırma sonucunda hemşirelerin istenmeyen olaylar karşılaşma durumlarının yarıdan az olduğu fakat karşılaşanların bildirim oranının düşük olduğu tespit edilmiştir. Hemşirelerin olay bildirimi eğitimi almaları, olay bildirim sistemi farkındalıkları ve kullanım bilgileri, olay bildirim tutumları, güvenlik tutumları ve olay bildirim engellerinin hasta güvenliğine yönelik istenmeyen olayları bildirme durumları üzerinde etkiye sahip olduğu belirlenmiştir Hemşirelerin bildirim sistemi kullanımını etkileyen faktörlerin göz önünde bulundurması, olay bildirim sonrasında yapılan iyileştirme faaliyetlerinde hemşirelerin sürece dâhil edilmesi ve geri bildirim yapılması, hemşirelerin olay bildirim tutumları, güvenlik tutumları ve olay bildirim engellerinin düzenli aralıklarla değerlendirilmesi önerilebilir. The research was conducted as a descriptive study in order to examine the nurses' use of event reporting systems and the factors affecting them. The population of the study consisted of 348 nurses working in a public hospital. The research data were collected between April 2022 and June 2022, with 264 nurses who agreed to participate in the study, "Demographic Information Form", "İncident Reporting Awareness and Usage Information Form", " Reporting of Clinical Adverse Events Scale (Rocaes) ", " Safety Attitudes Questionnaire (SAQ) " was collected by face-to-face interview technique using the "İncident Reporting Barriers Scale". Descriptive statistics, Pearson correlation coefficient, independent t tests, one-way analysis of variance (One-Way ANOVA) were used in the data analysis of the research. According to the results of the analysis, 44.7% of the nurses did not receive any incident reporting training, 39% of them witnessed or participated in an event related to patient safety that should be reported in the profession, however, only 37.9% reported an incident regarding patient safety. It was determined that 81.8% of the nurses had incident reporting system awareness and 55.3% had incident reporting usage knowledge. The RoCAES score of the nurses included in the study the mean was found to be 2.24±0.34 and their attitudes towards incident reporting were partially negative, the mean score of SAQ was found to be 3.44±0.50 and their safety attitudes were positive, the mean score of Incident Reporting Barriers Scale was found to be 2.70±0.72 and incident reporting obstacle perceptions were determined to be partially high. A statistically significant difference was found between the nurses' event reporting status and incident reporting attitudes, safety attitudes, and incident reporting barriers (p<0.05). As a result of the research, it was determined that the rate of encountering undesirable events was less than half of the nurses, but the reporting rate of those who encountered it was low. It has been determined that nurses receiving incident reporting training, incident reporting system awareness and usage information, incident reporting attitudes, safety attitudes and incident reporting barriers have an effect on reporting undesirable events for patient safety. It can be recommended to include nurses in the improvement activities or to provide feedback, and to evaluate the incident reporting attitudes, safety attitudes and incident reporting barriers of nurses at regular intervals.
