    GLICLAZID INDUCED THROMBOCYTOPENIA

    Background. Drug induced thrombocytopenia is mostly related with nonsteroidal anti-inflammatory drugs (NSAID), anticonvulsants, sulfonamides, diuretics, cinchona alkaloid derivatives, penicillamine and gold salts. Oral sulfonylureas such as glibenclamide, chlorpropamide and glimepiride are known to induce thrombocytopenia. Case report. We report a 42 year old female admitted to emergency department with a complaint of hematochesia. She has been using oral gliclazide for three years. Laboratory results revealed bicytopenia: haemoglobin=8.9 g/dL (N=12.3-15.3), white blood count (WBC)=12100/mu L (N=4400-11300). platelet count=4000/mu L (N=150000-450000). All examinations to etiology of thrombocytopenia were negative including, autoimmune, infectious (viral-bacterial) and haematological diseases. Colonoscopic examination showed 50% construction of the lumen ill the first 15 cm segment of the colon by all ulcerovegetant mass. Pathological examination was reported as adenocarcinoma. Thrombocyte levels increased oil the 4(th) day after stopping gliclazid treatment. Conclusions. It is the first case of gliclazid induced thrombocytopenia in literature. So we recommended that platelet count should be regularly checked ill all patients receiving sulfontylurea drugs including gliclazid.

    Detection of subclinical atrial dysfunction by two-dimensional echocardiography in patients with overt hyperthyroidism

    Background. - Hyperthyroidism is an important cardiovascular risk factor in the development of atrial fibrillation and heart failure. Increased atrial electromechanical intervals are used to predict atrial fibrillation, measured by tissue Doppler imaging (TDI). Aims. - To evaluate atrial electromechanical delay (EMD) and left atrial (LA) mechanical function in patients with overt hyperthyroidism. Methods. - Thirty-four patients with overt hyperthyroidism and 34 controls were included. A diagnosis of overt hyperthyroidism was reached with decreased serum thyroid-stimulating hormone (TSH) and increased free T4 (fT4) concentrations. Using TDI, atrial electromechanical coupling (PA) was obtained from the lateral mitral annulus (PA lateral), septal mitral annulus (PA septum) and right ventricular tricuspid annulus (PA tricuspid). LA volumes (maximum, minimum and presystolic) were measured by the disks method in apical four-chamber view and indexed to body surface area. LA active and passive emptying volumes and fractions were calculated. Results. - LA diameter was significantly higher in hyperthyroid patients (P = 0.001). LA passive emptying volume and fraction were significantly decreased in hyperthyroid patients (P = 0.038 and P < 0.001). LA active emptying volume and fraction were significantly increased in hyperthyroid patients (P < 0.001 and P < 0.001). Left and right intra-atrial (PA lateral-PA septum and PA septum-PA tricuspid) and interatrial (PA lateral-PA tricuspid) EMDs were significantly higher in hyperthyroid patients (29.2 +/- 4.4 vs 18.1 +/- 2.6, P < 0.001; 18.7 +/- 4.3 vs 10.6 +/- 2.0, P < 0.001; and 10.5 +/- 2.9 vs 7.1 +/- 1.2, P < 0.001, respectively). Stepwise linear regression analysis demonstrated that fT4 and TSH concentrations were independent predictors of interatrial EMD (beta = 0.436, P < 0.001 and beta = -0.310, P = 0.005, respectively). Conclusion. - This study showed prolonged atrial electromechanical intervals and impaired LA mechanical function in patients with overt hyperthyroidism, which may be an early sign of subclinical cardiac involvement and dysrhythmias in overt hyperthyroidism.     The Determination of Opinions of Triathlon Athletes on Doping and Anti-Doping Matters

    Objectives: This study aims to determine the knowledge of triathlon athletes on the types of doping used in sports as well as their opinions on the anti-doping matters. Methods: This descriptive study was conducted with 92 individuals participated in Tasucu Triathlon and Istanbul Salcano Triathlon Series, which was organized in 2015 by the Turkish Triathlon Federation. A questionnaire was used to collect data. Chi-square test was used in analysis of the data. Corticosteroids, masking agents, anti-estrogenic agents, beta blockers, and cannabinoids were among the types of doping which were least known by the triathlon athletes. These were followed by peptide hormones, narcotic analgesics, anabolic-androgenic steroids and stimulants. Results: Approximately two thirds of triathlon athletes consider that knowledge of athletes, coaches and administrators on doping is not sufficient. Conclusion: The majority of the athletes participated in the study stated that the relevant institutions and organizations are not fighting against doping adequately.

    Susceptibility Imaging in Glial Tumor Grading; Using 3 Tesla Magnetic Resonance (MR) System and 32 Channel Head Coil

    Background: Susceptibility weighted imaging (SWI) is a velocity compensated, high-resolution three-dimensional (3D) spoiled gradient-echo sequence that uses magnitude and filtered-phase data. SWI seems to be a valuable tool for non-invasive evaluation of central nervous system gliomas. Relative cerebral blood volume (rCBV) ratio is one of the best noninvasive methods for glioma grading. Degree of intratumoral susceptibility signal (ITSS) on SWI correlates with rCBV ratio and histopathological grade. This study investigated the effectiveness of ITSS grading and rCBV ratio in preoperative assessment. Material/Methods: Thirty-one patients (17 males and 14 females) with histopathogical diagnosis of glial tumor undergoing routine cranial MRI, SWI, and perfusion MRI examinations between October 2011 and July 2013 were retrospectively enrolled. All examinations were performed using 3T apparatus with 32-channel head coil. We used ITSS number for SWI grading. Correlations between SWI grade, rCBV ratio, and pathological grading were evaluated. ROC analysis was performed to determine the optimal rCBV ratio to distinguish between high-grade and low-grade glial tumors. Results: There was a strong positive correlation between both pathological and SWI grading. We determined the optimal rCBV ratio to discriminate between high-grade and low-grade tumors to be 2.21 Conslusions: In conclusion, perfusion MRI and SWI using 3T MR and 32-channel head coil may provide useful information for preoperative glial tumor grading. SWI can be used as an accessory to perfusion MR technique in preoperative tumor grading.

    Protection of Saphenous Vein Graft from Arterial Pressure An Experimental Study

    Background: Reoperations for bypass surgery increase the need for new grafts. We investigated early changes in both the normal human saphenous vein and in ectatic varicose veins externally supported by PTFE (polytetrafluoroethylene) graft and exposed to arterial pressure in an in vitro non-pulsatile flow model. Material and Methods: A total of 24 saphenous vein pieces (11 of them normal, the other 13 with varicosities) with a length of 6 centimeters were divided into equal parts: half of these parts were wrapped in PTFE grafts. All vein parts were placed in a perfusion circuit. Tissue biopsies were obtained from the vein segments. Light and electron microscopy examinations were performed, and endothelial continuity, elastic laminate continuity, medial connective tissue uniformity, medial smooth muscle uniformity, and adventitial connective tissue uniformity parameters were identified. Results: All parameters in the PTFE protected vein groups were better. The fewest morphological changes among all four groups were detected in the vein walls from normal veins with PTFE protection. There was no significant difference in endothelial continuity and adventitial connective tissue uniformity between the normal vein group and the varicose vein group with PTFE protection. Conclusions: It is suggested that supporting vein grafts externally with PTFE sufficiently protects the vein walls against damage from exposure to arterial pressure. If varicose veins are used as arterial grafts, supporting them with PTFE may be useful because of the good protection of endothelial and medial connective tissues, resulting in similar parameters to those of normal vein walls.

    Anti-edematous, anti-inflammatory and neuroprotective effect of etanercept in acute stage in experimental head injury

    BACKGROUND: To study the anti-edematous, anti-inflammatory, and neuroprotective effect of etanercept in the model of experimental head injury. METHODS: In this study, 40 male-adult Spraque-Dawley rats, with weight ranging from 250g to 300g, were used. The rats are divided into groups as control; non-penetrating trauma; trauma + NS; post-traumatic normal saline; trauma + D; post-traumatic dexamethasone and trauma + E. All medicines were given into peritoneum. After applying trauma and medicine, rats were decapitated in the 24th hour and the samples were studied histopathologically. RESULTS: In the study, a statistically significant difference was observed between the groups of trauma + NS and trauma dexamethasone according to the variables of edema and inflammation, but no difference was observed according to the variables of neuronal damage, astrocytic damage, and glial apoptosis. Moreover, a significant difference was observed between groups of Trauma + NS and trauma+ etanercept and between the groups of trauma + dexamethasone and trauma + etanercept in terms of all variables. CONCLUSION: It was observed that etanercept has anti-edematous, anti-inflammatory, and neuroprotective effect on the rats which experienced traumatic brain injury.

    Triatlon Atletlerinin Doping ve Anti-DopingKonusundaki Görüşlerinin Belirlenmesi

    Amaç: Bu çalışmanın amacı, triatlon atletlerinin sporda kullanılan doping türleri ve anti-doping konularındaki bilgilerini belirlemektir. Metot: Bu tanımlayıcı çalışma, Türkiye Triatlon Federasyonu tarafından organize edilen Taşucu Triatlon ve İstanbul Salcono Triatlon Serilerine katılan 92 bireysel katılımcı ile gerçekleştirilmiştir. Veriler anket kullanılarak toplanmıştır. Verilerin analizinde Ki-kare testi kullanılmıştır. Triatlon atletleri tarafından en az bilinen doping türleri arasında kortikosteroidler, maskeleyici ajanlar, anti-östrojenik ajanlar, beta blokerler ve kanabinoidler vardır. Bunları peptit hormonları, narkotik analjezikler, anabolik androjenik steroidler ve uyarıcılar takip etmiştir. Bulgular: Triatlon sporcularının yaklaşık üçte ikisi, sporcuların, antrenörlerin veyöneticilerin doping konusunda bilgilerinin yeterli olmadığını düşünmektedir.. Sonuç: Araştırmaya katılan sporcuların çoğunluğu, ilgili kurum ve kuruluşların dopingle yeterince mücadele etmediğini belirtmiştir.

    Effect of various irrigation solutions on cytokine expression of human gingival fibroblast: in vitro study

    Aim: The aim of this study is to investigate the release of IL-1,TNF- ? and VEGF following administration of NaOCl, propolisand chitosan solutions on human gingival fibroblasts.Materials and Methods: This study was conducted on humanfibroblast by cell culture and evaluation of the direct effectof various solutions on the cultured cells. The release ofpro-inflammatory interleukin (IL), tumor necrosis factor (TNF)? and vascular endothelial growth factor (VEGF) on fibroblastwas analyzed after administration of irrigation solutions. Logconcentrations of NaOCl, Propolis and Chitosan effects oncells were measured by colorimetric method.Results: IL-1 and TNF-? secretion levels decreased duringpropolis and chitosan applications, which are natural products.It was also found that the propolis increased VEGF secretionmore than the other materials.Conclusions: The results of this study suggest that propolisand chitosan may contribute to the recovery of periapical tissuesvia anti-inflammatory cytokines level secreted during theinflammatory process. It is important to search for the biologicaleffect of the materials in contact with the direct or indirectcause of the surrounding tissues during the endodontic treatment.
