Öğe Acute generalized exanthematous pustulosis associated with terbinafine: a case report(Informa Healthcare, 2013) Turan, Hakan; Acer, Ersoy; Erdem, Havva; Uslu, Esma; Aliağaoğlu, CihangirA 27-year-old male patient who has used oral terbinafine for two weeks was admitted to our outpatient clinic for non-follicular millimetric pustules on erythematous and edematous different-sized plaques on his trunk and flexural areas. He was diagnosed with acute generalized exanthematous pustulosis (AGEP) because of terbinafine use in the light of history, clinical and histopathological findings. An AGEP is a rare and severe pustular reaction usually triggered by systemic drug intake. Approximately, 2.3% of the patients having oral terbinafine have been reported to develop cutaneous adverse effects. Although terbinafine is a commonly used medicine, it must be considered that it may cause severe adverse reactions.Öğe Baboon syndrome induced by ketoconazole(Informa Healthcare, 2014) Güleç, Ali İhsan; Uslu, Esma; Başkan, Elife; Yavuzcan, Gizem; Aliagaoğlu, CihangirA 27-year-old male patient presented with a maculopapular eruption on the flexural areas and buttocks after using oral ketoconazole. The patient was diagnosed with drug-induced baboon syndrome based on his history, which included prior sensitivity to topical ketoconazole, a physical examination, and histopathological findings. Baboon syndrome is a drug-or contact allergen-related maculopapular eruption that typically involves the flexural and gluteal areas. To the best of our knowledge, this is the first reported case of ketoconazole-induced baboon syndrome in the English literature.Öğe Öğe Borrelia burgdorferi Enfeksiyonu ile Oluşan BirAkrodermatitis Kronika Atrofikans Olgusu(2015) Çalışkan, Emel; Uslu, Esma; Turan, Hakan; Kılıç, Nida; Aliağaoğlu, CihangirLyme hastalığı Borrelia burgdorferi'nin neden olduğu multisistemik bir zoonozdur. B. burgdorferi Spirochaetacea ailesinden gevşek kıvrımlı ve hareketli bir spiroket olup, İxodes cinsi sert kenelerin ısırması ile insanlara geçmektedir. Eritema migrans, akrodermatitis kronika atrofikans ve morfea benzeri lezyonlar, deri bulguları olarak karşımıza çıkabilmektedir. Geç dönem lezyonu olan ve sıklıkla kırk yaş üstü kadınlarda görülen akrodermatitis kronika atrofikans, kızarıklık ve şişlikle başlayıp aylar ya da yıllar içinde atrofi ve skleroza neden olabilmektedir. Bu raporda akrodermatitis kronika atrofikansa neden olan bir B. burgdorferi enfeksiyonunun sunulması amaçlanmıştır.Öğe A case of thoracoabdominal localized Mondor's disease in a young man [Genç erkekte torakoabdominal yerlesimli mondor hastali?i](Turkiye Klinikleri, 2014) Güleç, Ali İhsan; Yavuzcan, Gizem; Uslu, Esma; Başkan, Elife; Ünlü, Elif NisaMondor's disease is a special form of superficial thrombophlebitis of subcutaneous veins. It is more common in women. Penile Mondor's disease is generally detected in male patients. We reported a 30-year-old male patient having right thoracoabdominal wall Mondor's disease. The patient did not have any history of trauma, infection, physical activity and previous surgery. A linear, cord-like fibrous nodule with skin color was identified on the right thoracoabdominal wall in a diameter of 0.5 cm and length of 10 cm was detected in physical examination. The patient was given non-steroidal anti-inflammatory drug treatment. Mondor's disease should be kept in mind in the differential diagnosis of painful palpable cord-like lesions at thoracoabdominal wall in the presence of subcutaneous infiltration without any predisposing factors in healthy males. Copyright © 2014 by Türkiye Klinikleri.Öğe Cutaneous Larva Migrans: Report of Three Cases from the Western Black Sea Region, Turkey(Ankara Microbiology Soc, 2016) Çalışkan, Emel; Uslu, Esma; Turan, Hakan; Başkan, Elife; Kılıç, NidaCutaneous larva migrans (CLM) is a parasitosis frequently seen in persons who have travelled to tropical or subtropical regions and in those who have worked in contact with soil. The disease frequently develops due to Ancylostoma braziliensis and Ancylostoma caninum species. After penetrating the skin and entering the body, the hookworm larva proceeds to bore tunnels through the epidermis, creating pruritic, erythematous, serpiginous lesions. Secondary bacterial infections of the lesions can often be seen, especially on the legs and buttocks. In this article we presented three atypical local cases which have not been declared previously in our country. The first case, a 54-year-old male who was admitted to hospital in August with complaints of an obverse body rash and itching lasting for a week. Eruptions were observed over a small area on the right side of the abdomen, consisting of itchy, raised, erythematous, curvilinear string-like lesions. Moreover, no eosinophilia was detected in the patient, whose culture showed a growth of Streptococcus pyogenes. The patient was clinically diagnosed with CLM accompanied by secondary bacterial infection and treated for three days with 1 g of amoxicillin-clavulanic acid, mupirocin cream and albendazole 400 mg/d. Under this regime, the lesions were seen to decline. The second case, a 38-year-old male was also admitted in August, complaining of itching and redness on his body. The patient, whose blood count values were normal, exhibited itchy, raised, serpiginous string-like lesions located on the left side of his body. The patient, whose bacterial culture was negative, was clinically diagnosed as CLM and treated for three days with albendazole 400 mg/d and the lesions were seen to improve. The third case, a 23-year old male was admitted in September complaining of itching and redness on his neck. An itchy, crescent-shaped erythematous lesion was detected on his neck; bacteriological cultures and blood count were normal. The common feature for all three cases was the story of working in a hazelnut orchard and mowing weeds using a motorized string trimmer (weed whacker). None of them had a history of travel outside the country. Therefore CLM assumed to be occurred due to the aeration of surface earth layer with the force of motorized string trimmer and entrance of the larvae were from the open parts of the body. In conclusion, it should be keep in mind that hookworm larva-related CLM can be encountered in our country, and reporting of the patients with similar findings are necessary to determine the prevalence of this parasitosis in our country.Öğe Glutathione S-Transferase M1 and T1 Gene Polymorphisms in Patients with Chronic Plaque-Type Psoriasis: A Case-Control Study(Karger, 2016) Solak, Berna; Karkucak, Mutlu; Turan, Hakan; Ocakoğlu, Gökhan; Sağ, Şebnem Özemri; Uslu, Esma; Erdem, TeomanObjective: To determine the role of glutathione S-transferase (GST) isoenzyme polymorphisms as susceptibility factors in patients with psoriasis in a Turkish cohort. Subjects and Methods: In this case-control study, 105 patients with plaque-type psoriasis and 102 healthy controls were recruited from the dermatology outpatient clinics of two university hospitals. Genomic DNA was extracted from whole blood using a DZ DNA isolation kit. Multiplex PCR was used to determine GSTM1 and GSTT1 polymorphisms in the isolated DNAs. Results: Of the 150 patients with psoriasis, 83 (79%) were identified with the GSTT1 genotype and 22 (21%) with the null genotype. Of the 102 patients in the control group, 69 (67.6%) subjects were identified with the GSTT1 genotype and 33 (32.4%) with the null genotype. There was no significant difference between the patient and control groups (p = 0.063). Regarding the GSTM1 polymorphism, 54 (51.4%) patients were identified with this genotype and 51 (48.6%) with the null genotype; in the control group, 50 (49%) were identified with this genotype and 52 (51%) with the null genotype. Again there was no statistically significant difference between the groups (p = 0.957). Conclusion: In this Turkish cohort of patients with psoriasis, neither GSTT1 nor GSTM1 polymorphisms were associated with disease susceptibility. Larger studies with a wider range of GST isoenzyme are needed. (C) 2015 S. With the help of history, physical examination and histopathological features, the patient was diagnosed with iododerma secondary to topical povidone-iodine use. Iododerma develops frequently after oral or intravenous but rarely after topical use of iodine. Its pathogenesis is not well-known though it is widely believed that it is a delayed hypersensitivity reaction.Öğe Kütanöz Larva Migrans: Batı Karadeniz Bölgesinden Üç Olgu Sunumu(2016) Çalışkan, Emel; Uslu, Esma; Turan, Hakan; Başkan, Elife; Kılıç, NidaKütanöz larva migrans (KLM), sıklıkla tropikal ve subtropikal bölgelere seyahat eden ve toprakla temas öyküsü olan kişilerde görülen bir parazitoz olarak bilinmektedir. Kancalı kurt larvaları deriyi delerek vücuda girdikten sonra, epidermiste tüneller oluşturarak ilerlemekte ve kaşıntılı, eritematöz, serpijinöz lezyonlar meydana getirmektedir. Sıklıkla Ancylostoma braziliensis ve Ancylostoma caninum türlerine bağlı olarak gelişmektedir. İkincil bakteriyel enfeksiyonların da eklenebildiği lezyonlar özellikle ayak ve kalçada görülmektedir. Bu raporda ülkemizden daha önce bildirimine rastlamadığımız ve atipik yerleşimli üç olgu sunulmaktadır. Elli dört yaşında erkek hasta olan birinci olgu, Ağustos ayında gövde ön yüzünde bir haftadır olan kızarıklık ve kaşıntı şikayetiyle başvurmuştur. Karın sağ yanında bulunan, kaşıntılı, kavisli şekilde, deriden kabarık, eritemli, ip benzeri lezyonlarının küçük bir bölümünde sivilceleşme izlenmiştir. Kültüründe Streptococcus pyogenes üremesi de olan hastada eozinofi li saptanmamıştır. Hastaya mevcut klinikle KLM ile birlikte ikincil bakteriyel enfeksiyon tanısı konularak, amoksisilin-klavulanik asit 1 gr, mupirosin krem ve albendazol 400 mg/gün olmak üzere üç gün tedavi uygulanmıştır. Kontrolünde lezyonların gerilediği görülmüştür. İkinci olgu olan 38 yaşındaki erkek hasta, Ağustos ayında gövdede kaşıntı ve kızarıklık şikayetiyle başvurmuştur. Gövdenin sol tarafına yerleşmiş, kaşıntılı, serpijinöz karakterde, deriden kabarık, ip benzeri lezyonları bulunan hastanın hemogram değerleri normaldir. Bakteriyolojik kültürlerinde üremesi olmayan hastaya klinik olarak KLM tanısı konularak albendazol 400 mg/gün ile üç gün tedavi uygulanmış ve lezyonların gerilediği gözlenmiştir. Üçüncü olgu, 23 yaşında erkek hasta olup Eylül ayında boynunda kaşıntı ve kızarıklık şikayetiyle başvurmuştur. Boyunda yay şeklinde kaşıntılı, eritemli bir lezyon tespit edilen hastanın bakteriyolojik kültürleri ve hemogramı normaldir. Klinik olarak KLM teşhisi konulan hastaya albendazol 400 mg/gün ile üç gün tedavi uygulanmış ve lezyonlar tamamen gerilemiştir. Her üç olguda da fındık bahçesinde motorlu tırpanla ot biçme öyküsü mevcuttur ve yurt dışına seyahat öyküsü bulunmamaktadır. Dolayısıyla KLM'nin, motorlu tırpanın oluşturduğu güçle yüzeyel toprak tabakasının havalanması ve topraktaki larvaların vücudun açık kısımlarından girmesiyle oluştuğu kanısına varılmıştır. Recent reports have demonstrated that elevated high sensitive C-reactive protein(hsCRP), an inflammatory marker, and plasma ADMA levels are associated with endothelialdysfunction and increased atherogenesis. We aimed to determine the presence of endothelialdysfunction in psoriatic patients and if psoriazis is an independent risk factor for cardiovasculardiseases by measuring the serum levels of ADMA and hsCRP in patients with psoriazis.Material and Methods: Thirty five psoriatic patients not having traditional cardiovascular riskfactors and 26 healthy controls were included into the study. Serum levels of ADMA, hsCRP,HDL, LDL, triglyceride, and total cholesterol, and BMI analyses were assessed and comparedbetween patient and control subjects. The association of psoriazis area and severity index (PASI)scores with ADMA and hsCRP levels were evaluated. Results: There was no statistically significant difference for the demographic characteristics,and all laboratory parameters levels between psoriazis and control groups. ADMA values were0.63±0.30 ?mol/L (mean±sd) and 0.68±0.45 ?mol/L (mean±sd) in patients and control group,respectively. hsCRP levels were 1.40±1.51 mg/L (mean±sd) in control group, while 1.91±2.13mg/L (mean±sd) in patients group. There was no statistically significant difference betweenpatients and control groups, in terms of ADMA and hsCRP values (p gt;0.05). MBL2 gene polymorphisms affect MBL serum levels. Therefore, this increases the risk of infection and may result in predisposition to autoimmune diseases. The aim of this study was to investigate whether there is an association between the MBL2 gene codon 54 (allele B: rs1800450, c. 161G>A; p. 54Gly>Asp) polymorphism and vitiligo in Turkish patients. One hundred and one patients who were diagnosed with vitiligo and 101 control subjects were included in the study. The DNA was analyzed using the Kbioscience Competitive Allele Specific PCR (KASP) technique. MBL2 gene codon 54 polymorphism frequencies were compared between the two groups. In statistical analysis, the level of significance was set at p<0.05. No significant differences in frequencies of the A allele were observed between the patient and control groups. It was observed at similar frequencies in both groups (p=0.890). The results suggest that the MBL2 gene Codon 54 polymorphism is not associated with an increased risk for the development of vitiligo in Turkish patients.Öğe Nonvenereal sclerosing lymphangitis of the penis: Scientific letter [Penisin nonveneryal sklerozan lenfanjiti](2012) Turan, Hakan; Büyükkaya, Ramazan; Uslu, Esma; Kayıkçı, Muhammet AliNonvenereal sclerosing lymphangitis is a rarely seen disease characterized by traumatic obstruction of big lymphatic vessels of penis after a hard sexual intercourse. A 44-year-old male patient applied to our outpatient clinic for a rope-like thickening on his penis. His history revealed a trauma to the penis during sexual intercourse 1-2 days ago. There was no history of any drug use. A firm, skin-colored, cord-like lesion located perpendicular to the long axis of the penis and completely surrounding the dorsal coronary sulcus of the penis was detected on the dermatological examination. The complete blood count, blood chemistry, urinalysis values were normal; rapid plasma reagin, HIV, and hepatitis markers were negative. Penile Doppler ultrasonography was found to be normal. The patient was diagnosed as nonvenereal sclerosing lymphangitis by current clinical and laboratory findings. We think that it is quite important to diagnose this rarely seen disease especially by dermatologists and urologists to avoid misdiagnosis, unnecessary laboratary examinations and treatment. Otuz yıldır olan diyabete bağlı oluşan periferik nöropati nedeniyle krutlu uyuz lezyonları kaşıntısız idi. Potasyum hidroksit ile alınan deriörnekleri mikroskobik incelemesinde çok sayıda uyuz akarları ve yumurtalarını saptandı. Birer haftalık Topikal% 5 permetrin krem tedavisi iki kür olarak hastaya olarak reçete edildi.Öğe Periorbital discoid lupus erythematosus: A case report(Aves Press Ltd, 2016) Uslu, Esma; Başkan, Elife; Başar, Feyza; Turan, Hakan; Oktay, MuratDiscoid lupus erythematosus (DLE) is the most frequent form of cutaneous lupus erythematosus. It is typically located on sun-exposed areas, such as forehead, nose, cheeks, eyelids, scalp, neck, presternal area, and the extensor aspects of extremities. Eyelid involvement is rare and usually it is accompanied with skin lesions elsewhere on the body. On this localization, DLE can mimic blepharoconjunctivitis, allergic dermatitis, psoriasis, lichen planus, basal cell carcinoma, granuloma annulare, tinea faciale, and eyelid edema. On the periorbital region, therefore, if one of these diseases is pre-diagnosed but there is no response to the treatment, DLE should be considered in the differential diagnosis. Here, the case of a 25 year-old male patient with discoid lupus erythematosus that has a peri-orbital location is presented, along with the response to topical tacrolimus cream treatment.Öğe Piebaldism: A familial case report [Piebaldizm: Ailesel bir olgu sunumu](2013) Turan, Hakan; Uslu, Esma; Acer, Ersoy; Gün, Emrah; Başar, FeyzaPiebaldism is a rare autosomal dominant skin disease presented with white forelock and depigmented patches. It is characterized by congenital absence of melanocytes and caused by mutation of KIT-proto-oncogene. KIT gene encodes transmembranous receptors on the surface of the melanocytes and it is responsible for the migration, proliferation, and differentation of the melanoblasts. The exact prevalence is unknown. It affects the both sexes equally. Here, we present a 3-year-old case admitted by depigmented macules, patches, and white forelock on forehead, trunk, upper and lower extremities. He had no accompanying systemic diseases and was diagnosed as piebaldism. His mother and older sister had similar lesions. On the occasion of this case, we aimed to create an awareness about piebaldism which may be confused especially with vitiligo clinically. Klinik olarak kaşıntılı eritemli papül ve papüloveziküller ile karakterizedir. Lezyonlar genellikle gövdeye, sırtın üst kısmına, sakral bölgeye, karına ve göğüse yerleşir ve postinflamatuar hiperpigmentasyon ile iyileşir. Sırtta ve gövde ön yüzünde kahverengi lekeler ve kaşıntı hikayesiyle polikliniğimize başvuran 22 yaşındaki bir kadın olguya, klinik ve histopatolojik özellikleri ile prurigo pigmentosa tanısı konuldu. Prurigo pigmentosanın nadir görülen bir hastalık olması ve olgunun doksisiklin tedavisine cevap vermesi nedeniyle sunulmaya değer bulunmuştur.Öğe Psöriyazislere Mevsimsel Bakış(2015) Erdem, Havva; Başar, Feyza; Kadıoğlu, Nilüfer; Bahadır, Anzel; Uslu, Esma; Yavuzcan, Gizem; Oktay, MuratPsöriyazis, dünya üzerinde popülasyonunun %2-3ünü etkileyen kronik inflamatuar bir hastalıktır. Etyopatogenezi hala tam olarak aydınlatılamamıştır. Kadın ve erkekler eşit oranda etkilenirler. Çalışmada, 142 vaka erkek,121 vaka kadındır. Erkek vakalarda ortalama yaş 42,5 (min-maks: 1-81), kadınlarda ortalama yaş 37,9dur (min-maks: 2-82). İstatistiksel analiz sonucunda, ilkbahardaki vaka sayılarının ortalaması, yaz mevsimindeki vaka sayıları ortalamalarından anlamlı derecede daha yüksek bulunmuştur (p0,044). Diğer mevsim grupları arasında ise vaka sayıları ortalamaları bakımından istatistiksel olarak anlamlı bir farklılık gözlenmemiştir (p0,05). Vaka sayıları bakımından mevsimsel dağılım açısından anlamlı fark yoktur. Bu durumda, mevsimlerdeki vaka sayılarının benzer olduğu söylenebilir (p0,05). Dexpanthenol is the stable alcoholic analogue of pantothenic acid. It is widely used in cosmetics and topical medical products for several purposes. We present the case of 8-year-old girl with pustules over erythematous and eczematous areas on the face and neck. To the best of our knowledge, this is the first case reported that is diagnosed as pustular irritant contact dermatitis caused by dexpanthenol.
