    A Case of Nephrotic Syndrome With Pneumocystis Jirovecii Infection

    Pneumocystis jirovecii pneumonia (commonly called Pneumocystis pneumonia or PCP) is an opportunistic infection that occurs in immunocompromised individuals. 26 year-old male patient admitted to nephrology department for hypervolemic hyponatremia and consulted to our clinic due to the desaturation. He has been diagnosed with collapsing glomerulonephritis and he was using cyclosporine and prednisolone. Postero-anterior chest X-ray showed a blunt left cardiodiaphragmatic sinus. After ten days the patient's hypoxia deepened. Repeated chest X-ray showed bilateral perihilar heterogeneous opacity. Pneumocystis jirovecii was detected in lavage culture. We presented a case of Pneumocystis Jirovecii pneumonia secondary to cyclosporin toxicity because of a rare case.

    Clinical and polysomnographic evaluation of sleep-related breathing disorders in patients with sarcoidosis

    Objective This study aimed to evaluate the frequency of obstructive sleep apnea (OSA) in patients with sarcoidosis and related clinical factors. Materials and method Consecutive patients diagnosed with sarcoidosis in our clinic were evaluated for OSA risk during sleep using the Epworth Sleepiness Scale, Stanford Sleepiness Scale, Pittsburgh Sleep Quality Index, Berlin questionnaire, STOP and STOP-BANG questionnaires, and polysomnography (PSG). Results A total of 60 sarcoidosis patients (mean age: 50 +/- 11 years, 45 (75%) women) were included in the study. Polysomnography was performed in 54 cases and revealed the diagnosis of OSA in 70% (38/54) of the patients. The mean age was higher in patients with sarcoidosis and OSA (54 +/- 11 vs. 47 +/- 13, p = 0.041) and body mass index values were significantly higher as well (31.9 +/- 4.4 vs, 29.0 +/- 4.6 kg/m(2), p = 0.034). Polysomnography revealed a higher rate of OSA in patients with sarcoidosis who had high-risk scores in Pittsburgh Sleep Quality Index, STOP questionnaire, and STOP-BANG questionnaire (p = 0.024, p < 0.001, and p < 0.001, respectively). Based on polysomnography, OSA was detected in 39% (5/13) with stage 1 sarcoidosis, 78% (28/36) with stage 2, and in all cases (5/5) with stage 3. OSA frequency and apnea-hypopnea index (AHI) were determined to increase with advanced sarcoidosis stage (p = 0.003, p = 0.043, respectively). AHI was positively correlated with sarcoidosis stage (p = 0.003, r = 0.391). The prevalence of OSA was significantly higher in patients receiving treatment compared to treatment-naive patients (88% vs. 57%, p = 0.018). Multivariate logistic regression analysis revealed the stage of the disease (p = 0.026) to be the single independent risk factor associated with increased risk of OSA in patients with sarcoidosis. Conclusion High rates of OSA were detected in sarcoidosis, increasing with the advanced disease stage. The findings suggest that patients with sarcoidosis and advanced age, obesity, steroid treatment, and involvement of lung parenchyma (stages 2 and 3) should be evaluated for OSA risk. Further investigations are needed to establish the potential causes of the high prevalence of OSA in sarcoidosis.

    The Effect of Obstructive Sleep Apnea on Violent and Non-violent Behavior

    Aim: Obstructive sleep apnea (OSA) characterized by repetitive collapse of the upper airway during sleep and thiscondition leading to oxygen desaturation, sympathetic activation, and recurrent arousals. Patients who experience sleepproblems consider themselves, less able to control impulsive, aggressive tendencies. The aim of this study was toinvestigate the effect of OSA and daytime sleepiness on violent and non-violent behaviors.Material and Methods: Hundred fifty individuals who were admitted to the Chest Disease Polyclinic for SleepDisorders of Duzce University, School of Medicine Hospital were included in the study. All patients underwentpolysomnography (PSG). All tests [Beck Depression Inventory (BDI), Beck Anxiety Inventory (BAI), EpworthSleepiness Scale (ESS) and Nonviolent and Violent Offending Behavior Scale (NVOBS)] were applied to allparticipants by face to face interview.Results: In this study, no significant relationship was found between NVOBS and AHI in OSA patients. Patients withOSA; There was a significant relationship between total score of NVOBS and ESS (r=0.267 p=0.003). There was asignificant relationship between the ESS score and the BDI score (r=0.314 p <0.001) and BDI scale (r=0.319 p <0.001)scores.Conclusions: There was no significant relationship between violent and non-violent behaviors and AHI. However,there was a significant relationship between ESS and BDI, BAI and NVOBS. There may be commonneurophysiological activation mechanisms of sleep and violence action. Today, violent behavior and sleep problemsincrease and further research is needed to investigate the relationship between sleep problems and violent behavior.

    Monocyte/HDL ratio in sarcoidosis patients without treatment

    Aim: This study examined the effects of monocyte/high-density lipoprotein cholesterol (HDL-C) ratio (MHR) and other inflammatorymarkers in patients with sarcoidosis not receiving treatment. To our knowledge, this is the first study on the relationship betweensarcoidosis and MHR.Material and Methods: This study included 53 patients with sarcoidosis who were followed at a single, outpatient tertiary-carecentre. Data on patient demographics (age and gender), disease characteristics (duration of disease, radiographic stage, treatments);pulmonary function tests (% predicted values for forced vital capacity [FVC], forced expiratory volume at 1 second [FEV1], diffusingcapacity of the lungs for carbon monoxide [DLCO], alveolar volume [VA], forced expiratory flow [25–75% and at 50%; FEF] [FEF25-75]),blood biochemistry and hemogram findings were retrieved from hospital records.Results: Mean ± SD patient age was 51.13 ±1 1.94 years. Of the 53 patients, 42 were female and 11 were male. When compared withMHR ratio and pulmonary function test parameters, there was a negative correlation with FEV1% (P = 0.043, r = ?0.279) and DLCO% (P= 0.009, r = –0.360). There was no significant correlation between FVC, FEV25-75 or VA. No significant correlation was found betweenblood calcium, urinary calcium, SACE (serum angiotensin converting enzyme) values and MHO ratio.Conclusions: Large-scale prospective studies are needed to determine if the use of MHR as an indicator of inflammation andactivation would be helpful in the diagnosis and monitoring of patients with sarcoidosis.

    NADİR GÖRÜLEN BİR SENDROM: SWYER JAMES MACLEOD OLGUSU

    Swyer James MacLeod Sendromu (SJMS) özellikle çocukluk çağında sık pulmoner enfeksiyon sonrası geliştiği düşünülen radyolojik olarak tek taraflı radyolüsen görünümü olan nadir bir sendromdur. Genellikle asemptomatik olup insidental olarak saptanır. Unilateral radyolüsensi yapacak nedenler ekarte edildikten sonra radyolojik olarak tek taraflı hava hapsi, hiperlüsensi bulguları, küçük ya da normal boyutlarda akciğer, pulmoner damarların sayısı ve boyutlarında azalma karakteristik özellikleridir. Swyer James MacLeod Sendromu tedavisi, çoğu vakada konservatif olup hastaların enfeksiyonlara karşı korunması, grip ve pnömokok aşılarını yaptırmaları önerilmektedir. Burada 27 yaşında erkek olgumuzu nadir görülen bir sendrom olması nedeniyle sunmaktayız.

    Nadir Görülen Kartagener Sendromlu Bir Olgu

    Kartegener sendromu bronşiektazi, kronik sinüzit ve situs inversus triadı ile karakterize olanotozomal resesif geçişli, yaklaşık 30000 canlı doğumda bir görülen nadir bir hastalıktır.Elektron mikroskopik görüntülemede görülen en sık bozukluk, epitelyal silyalarda dyneinkollarının yokluğudur. Bu hastalıkta siliyal aktivitenin bozulması nedeniyle aşırı balgambirikmesi görülür. Hastalığın tedavisi semptomlara yöneliktir. İnfertilite ve sağırlık görülebilir.Kartagener sendromunun uyku kalitesini düşürdüğü de görülmüştür. Hastalarınenfeksiyonlardan korunması için göğüs fizyoterapisi, influenza ve pnömokok aşılarınınyapılması önerilir. Bu hastaların toz, duman gibi partikül içeren gazlardan uzak durmalarıgerekmektedir. Bu çalışmada, sık hastane başvurusuna rağmen geç tanı konulan, obstrüktifuyku apne sendromu ile birliktelik gösteren 33 yaşında erkek olgu klinik ve radyolojik bulgularile sunuldu.

    Obstrüktif Uyku Apnesi’nin Şiddet İçeren ve İçermeyen Davranışlar Üzerine Etkisi

    Amaç:Obstrüktif uyku apne (OUA) üst hava yollarının uyku sırasında daralması ilekarakterizedir ve bu durum oksijen desaturasyonu, sempatik aktivasyon,tekrarlayan arousallara neden olur. Uyku problemi yaşayan insanlar kendileriniagresif eğilimleri olan ve dürtüselliklerini daha az kontrol edebilen kişilerolarak düşünürler. Bu çalışmanın amacı OUA ve gündüz uykululuk halinin, şiddetiçeren ya da içermeyen davranışlar üzerine etkilerini araştırmaktır.Gereçve Yöntemler: Düzce Üniversitesi Tıp FakültesiHastanesi Göğüs Hastalıkları Polikliniğine uyku bozuklukları nedeniyle başvuran150 kişi çalışmaya dâhil edildi. Tüm hastalara polisomnografi (PSG) yapıldı.Tüm diğer testler [Beck depresyon ölçeği (BDÖ), Beck anksiyete ölçeği (BAÖ),Epworth uykululuk skalası (EUS) ve Şiddet içeren ve içermeyen suç davranışlarıölçeği (ŞİSDÖ)]  katılımcılarla yüz yüzegörüşme ile yapıldı.Bulgular:Bu çalışmada OUA hastalarında ŞİSDÖ ile apne-hipopneindeksi (AHI) arasında anlamlı bir ilişki saptanmadı. OUA’li hastalarda toplamŞİSDÖ skoru ile EUS arasında anlamlı bir ilişki bulundu (r=0,267 p=0,003). EUSile BDÖ skoru (r=0,314 p lt;0,001) ve BAÖ skalası skoru (r=0,319 p lt;0,001)arasında anlamlı bir ilişki vardı. Sonuç:AHI ile şiddet içeren ve şiddet içermeyen davranışlar arasında bir ilişkiyoktu. Bununla birlikte ESS ile BDÖ, BAÖ, ŞİSDÖ arasında anlamlı bir ilişkivardı. Uyku ve şiddet davranışlarının ortak nörofizyolojik bir mekanizması olabilir. Çalışmamızgündüz uykululuk halinin artmasıyla şiddet eğiliminin arttığını göstermiştir. Uykuproblemleri ile şiddet davranışlarının arasındaki ilişkiyi araştırmak içinileri çalışmalara ihtiyaç vardır.

    Relationship between Diagnosis Period and Internal and External Air Quality in Patients with Tuberculosis

    Objective: The aim of this study was to investigate the relationship between bacteriological case definitionsand indoor and outdoor air quality parameters in tuberculosis (TB).Materials and Methods: A total of 200 patients with TB diagnosed and treated in our hospital during 2012-2018 were included to this study. The air monitoring measurement parameters of the National Air QualityNetwork [particulate matter 10 (PM10), sulfur dioxide (SO2), air temperature, air pressure, and relativehumidity] for the same time period were obtained from the web page http://laboratory.cevre.gov.tr/Default.ltr.aspx.Results: Of the 200 patients, 62.5% (125) were males and 37.5% (75) were females. The rate of diagnosisbased on culture and smear positivity was 48.4% (31), which was significantly higher than that in the clinic[10.9% (7)] among patients who used stove for warming. The rate of diagnosis based on culture and smearpositivity [52.1% (25)] was significantly higher than that in the clinic [8.3% (4)] among patients who were exposed to biomass. The univariate analysis revealed no significant independent effect of warming and biomassuse on case definition. According to the case definitions, the mean values of PM10, SO2, and temperature inthe diagnosed month showed no statistically significant difference. The humidity level in the month was significantly higher, during which cases diagnosed using smear and culture positivity were compared with casesdiagnosed using only culture positivity (p=0.023).Conclusion: This study indicates that biomass used as a cooking fuel is a risk factor for pulmonary TB, implying that TB occurrence can be reduced significantly by lowering or preventing the exposure to cookingsmoke emitted from biomass fuel.

    Secondary Brain Abscess in a Patient Followed Up with Lung Abscess

    Respiratory tract infections are common in Down syndrome. A 24-year-old male patient with Down syndrome with concurrent diabetes mellitus underwent antibiotic treatment with a diagnosis of lung abscess in an external center with complaints of fever and vomiting, but was referred to us after his symptoms did not regress. Despite the improvement in the lung abscess noted in a radiological examination, the patient was identified with a brain abscess upon an examination due to the continuation of fever, vomiting and the onset of headache. Particular attention should be paid to additional abscess foci such as brain abscess in cases with lung abscess with an underlying comorbidity.
