Öğe Association of vitamin D insufficiency/deficiency with thyroid artery Doppler ultrasonography in patients with Hashimoto thyroiditis(Professional Medical Publications, 2017) Nalbant, Ahmet; Aydın, Ayhan; Karacan, Alper; Önmez, Attila; Tamer, Ali; Cinemre, HakanBackground (sic) Objective: During the course of the autoimmune thyroid diseases, ultrasonography change parallel to histopathology. Vitamin D is associated with autoimmune diseases and thus can affect thyroid blood flow. Our aim was to investigate the relationship between vitamin D insufficiency/deficiency and thyroid hemodynamic indices in patients with Hashimoto thyroiditis. Methods: A total of 93 patients who presented to Sakarya University Endocrinology outpatient clinic from April to September 2016 and diagnosed with Hashimoto thyroiditis were included in this study. Clinical and serologic data, thyroid antibodies and 25(OH)D3 were evaluated. Mean peak systolic velocity(mPSV), mean end-diastolic velocity (EDV), mean resistive index (RI) flows of superior and inferior thyroid arteries were measured with B-mode Doppler ultrasonography. Results: Vitamin D insufficiency/deficiency was detected in 59 (63.4%). TPO Ab and TgAb levels were found higher in patients with vitamin D insufficiency/deficiency. In the normal vitamin D group, superior thyroid artery mPSV (32.21 +/- 6.73cm/s) and EDV(13.27 +/- 2.80 cm/s) were higher than in the low vitamin D group [mPSV (28.32 +/- 8.99cm/s) and EDV(10.67 +/- 3.68 cm/s)] (P=0.034, P=0.001, respectively). Inferior thyroid artery EDV value was higher in the normal compared to the low vitamin D group (0.032). RI measured in all arteries were higher in the vitamin D insufficient/deficient group compared to the Vitamin D normal group (p=0.001). Conclusion: Vitamin-D insufficiency/deficiency has led to reduced parenchymal blood supply and increased micro-vascular resistance in Hashimoto thyroiditis patients.Öğe Bir Üniversite Hastanesine Başvuran Göçmen ve Mülteci Hastaların Değerlendirilmesi(2021) Önmez, Attila; Gamsızkan, ZerrinAmaç: Bu çalışmanın amacı, Düzce Üniversitesi Araştırma ve Uygulama Merkezi’ne başvuran göçmen hastaların başvuru tanılarını ve hasta özelliklerini değerlendirmektir. Gereç ve Yöntemler: Çalışma retrospektif olarak hasta kayıtlarının incelenmesi şeklinde tasarlanmıştır. Çalışmada 2014-2019 yılları arasında Düzce Tıp Fakültesi Araştırma ve Uygulama Merkezi’ne başvuran göçmen ve mülteci hastaların kayıtları geriye dönük olarak incelenmiştir. Bulgular: Çalışma sonuçlarına göre son 5 yılda Düzce Üniversitesi Araştırma ve Uygulama Merkezi’ne 4754 göçmen ve mülteci hasta başvurusu olduğu belirlenmiştir. Mükerrer başvurular elendiğinde 834 göçmen ve mülteci hastanın beş yıl içinde ilimizdeki üçüncü basamak sağlık hizmetini kullandığı tespit edilmiştir. Hizmet alan 834 hastanın 440’ı kadın, 394’ü erkektir. Cinsiyete göre yaş grupları arasında anlamlı bir fark bulunmamıştır. En sık ziyaret edilen klinikler incelendiğinde başvuruların %15,6’sı (n=745) acil servis polikliniğine, %11,8’i (n=561) kadın hastalıkları ve doğum kliniğine, %9,7’si (n=464) çocuk sağlığı ve hastalıkları kliniğine olacak şekilde sıralandığı tespit edilmiştir. Sonuç: Çalışma Sonuçları ile Göçmen ve mülteci hastaların sağlık ihtiyaçlarının öncelikleri belirlenmiş olup, sağlık hizmetinin planlanmasına rehberlik etmektedir. Sağlık sisteminin tüm basamaklarında göçmen ve mülteci hastaların sağlık hizmeti kullanım verilerinin araştırılması ve değerlendirilmesi gerekmektedir.Öğe CAN WE USE THE TRIGLYCERIDE/HDL RATIO TO DETERMINE INSULIN RESISTANCE IN OBESITY SCREENING AND FOLLOW-UP IN PRIMARY CARE?(Ankara Yıldırım Beyazit University, 2022) Gamsızkan, Zerrin; Önmez, Attila; Şahin, İ.E.; Erdemir, G.; Ünal, S.Objectives: Obese patients are followed up with periodic laboratory tests. Insulin resistance is also a parameter used in these tests. There is a need for parameters that can determine insulin resistance more easily and practically. The aim of our study is to investigate the discriminative power of triglyceride (Tg) and triglyceride/high-density lipoprotein cholesterol ratio (Tg/HDL) for Homeostatic Model Assessment for Insulin Resistance (HOMA-IR) in our patient population by examining the laboratory findings. Materials and Methods: In this retrospective study, the laboratory data of patients who applied to the Family Medicine Obesity Polyclinic of Düzce University Hospital between April 2018 and April 2019 with the desire to lose weight and/or have healthy lifestyle suggestions were examined. Results: Of the 512 patients included in our study, 66.40% (n=340) were female and 33.59% (n=172) were male. In the examination performed on patients grouped according to their Body Mass Index (BMI), Total cholesterol, HDL, Low-density lipoprotein (LDL), Tg, HOMA-IR, Insulin and Tg/HDL ratios were significantly different between the groups. The cut-off value in the ROC analysis of the Tg/HDL parameter between patients with and without insulin resistance was found to be 2.29 with 66% sensitivity and 55% specificity (AUC: 0.634, p<0.001). In Spearman's correlation analysis of Tg/HDL and HOMA-IR, both parameters were found to have a statistically significant correlation (r: 0.248; p< 0.000). Conclusion: The significant relationship between the high Tg/HDL ratio and HOMA-IR detected in the results of our study shows that the Tg/HDL ratio can be used as a practical tool to evaluate insulin resistance in obese patients. © 2022 Universidade Estadual de Mato Grosso do Sul. All Rights Reserved.Öğe Cases of allergic coronary syndrome (Kounis syndrome) : what we should know(Nagoya Univ, Sch Med, 2022) Akbaş, Türkay; Kaya, Adnan; Altun, Gülşah; Esbah, Umit; Önmez, AttilaKounis syndrome (KS) is an acute coronary syndrome that occurs with allergic, hypersensitivity, anaphylactic, or anaphylactoid reactions associated with mast cell activation, and entails significant morbidity and mortality risks. We present four cases of acute coronary syndrome developing after insect bites, chemotherapy, and coronary stent implantation. Two patients were lost due to anaphylactic shock-related multiorgan failures and sudden cardiac death. Since a wide range of drugs, foods, environmental conditions, and disease states may be associated with KS, all physicians must be aware of the syndrome.Öğe Clinical assessment of patients with acromegaly(Wolters Kluwer Medknow Publications, 2018) Gökosmanoğlu, Feyzi; Önmez, AttilaBackground: In this study, we aimed to retrospectively evaluate treatment outcomes and treatment methods in acromegaly patients. Materials and Methods: The study included 65 acromegaly patients followed in Sakarya University Faculty of Medicine Department of Endocrinology in Turkey between 2004 and 2013. Clinical, biochemical, and radiological data were obtained retrospectively from the medical files of the patients. All cases have been discussed in the endocrinology and pituitary surgery council, and a multidisciplinary treatment approach had been chosen in management. Results: Sixty-five patients were included in the study. Of the patients, 57% were female. Mean age was 45.3 +/- 9.2 years old. Of the cases, 12.3% were microadenomas (n = 8, tumor diameter < 10 mm) and 87.6% were macroadenomas (n = 57, tumor diameter = 10 mm). In our study, 70% remission was achieved with the first operation and medical treatment. Patients with invasive acromegaly without remission after the first operation underwent reoperation, medical treatment, and conventional or stereotactic radiotherapy and achieved 45% remission rate. Conclusion: Pituitary surgery is the first treatment option for acromegaly. In patients who could not be remissioned after the first operation, remission can be achieved by combined therapy consist of reoperation, medical treatment, and conventional or stereotactic radiotherapy.Öğe A comparison of tetanus antibody levels between patients with high body mass indices and normal patients(2021) Çalışkan, Emel; İnce, Nevin; Önmez, AttilaAim: It has recently been thought that obesity may cause metabolic diseases as well as being linked to immune system dysfunction. The purpose of this study was to compare tetanus antibody levels between patients with high body mass indices and healthy-weight individuals and to examine the effect of obesity on tetanus antibody levels in adults. Materials and Methods: This cross-sectional research was performed at Duzce University,Faculty of Medicine from December 2018 to February 2019. The study group was composed of obese patients who had undergone tetanus immunization within the previous 10 years. The control group consisted of healthy-weight individuals who had also been immunized in the previous 10 years. An enzyme-linked immunosorbent assay kit (Euroimmun, Germany) was used to detect antibodies to tetanus toxoid. Results: Sixty-seven individuals with obesity and 21 controls participated in this study. Anti-tetanus IgG antibodies were at protective levels in both groups, although the mean antibody level in the patients with obesity was significantly lower compared to the control group (0.788±0.602 vs 1.112±0.398, p=0.022). Anti-tetanus IgG antibodies were significantly negatively correlated with BMI (r=- 0.269, p=0.016). Conclusion: The detection of low levels of tetanus antibody titers in patients with high BMI in this study compared to the control group suggests that greater attention is required in this population.Öğe Determination of cut-off values and sensitivities of tests for the diagnosis of subclinical cushing's syndrome in functional investigation of adrenal incidentalomas(Bayrakol Medical Publisher, 2019) Yıldırım, Abdurrahim; Gökosmanoğlu, Feyzi; Önmez, AttilaAim: The aim of our study was to determine the sensitivities of tests used in the diagnosis of subclinical Cushing's syndrome (SCS) in patients with adrenal incidentalomas using clinical findings and other biochemical parameters. Material and Method: One hundred and twenty-nine patients with adrenal incidentalomas who were followed up by our endocrinology clinic were included in the study. Patients were divided into 4 groups according to the 1-mg DST results and these groups were compared in terms of age, gender, hormonal parameters, and comorbidities associated with Cushing's syndrome. The cut-off value of salivary cortisol level for the diagnosis of SCS was calculated by ROC curve analysis. Result: If the cut-off values of plasma cortisol levels after the 1-mg DST were considered >= 1,8 mu g/dL, >= 3 mu g/dL, and >= 5 mu g/dL, the detection rates of SCS were respectively 22,4%, 1,4%, and 3,8%. When the cut-off value of plasma cortisol levels after the 1-mg DST was considered >= 1,8 mu g/dL, the sensitivity, specificity, and cut-off value of salivary cortisol levels were calculated as 68%, 57%, and 0.195 mu g/dL by ROC curve analysis. Discussion: As the adrenal mass size and patient age increased, cortisol suppression decreased according to the 1-mg DST. As the cut-off value of plasma cortisol level used in the 1-mg DST decreased, the incidence of SCS increased significantly. This leads to overdiagnosis of SCS in asymptomatic patients with adrenal incidentalomas. New cut-off values should be determined for salivary cortisol level according to different populations.Öğe The effects of vildagliptin in liver functions in type 2 diabetic patients(Bayrakol Medical Publisher, 2019) Gökosmanoğlu, Feyzi; Önmez, Attila; Topkaya, SibelAim: In Type 2 diabetic patients, the prevalence of liver disease is high when compared with the general population. For this reason, the hepatic safety of anti-diabetes agent is important. However, there are uncertainties about the long-term efficiency and reliability of vildagliptin treatment. In this study, we have reviewed the findings obtained from the clinical use of vildagliptin and its effects on the liver. Material and Method: The study was conducted on 243 patients who were followed-up in our clinic between September 2016 and May 2018. The data were obtained by reviewing the patient files retrospectively. Result: A decrease was detected in the aminotransferases after the treatment with vildagliptin, which was statistically significant (ALT: decreased from 78 +/- 17 to 48 +/- 14 IU/L (p = 0.029), AST: decreased from 63 +/- 13 to 41 +/- 11 IU/L (p=0.035); GGT: 20.5 +/- 3.2 and 19.1 +/- 6.3 (p=0.682)). Vildagliptin reduces the insulin resistance and Body Mass Index in patients. Discussion: It was determined that Vildagliptin is a safe treatment option for Type 2 Diabetes management.Öğe Influence of flexible insulin dosing with carbohydrate counting method on metabolic and clinical parameters in type 1 diabetes patients(Open Access Macedonian Journal of Medical Sciences, 2018) Gökosmanoğlu, Feyzi; Önmez, AttilaOBJECTIVE: The purpose of providing and maintaining a proper metabolic control is to prevent the development of chronic complications. In this study, we aimed to determine the influence of flexible insulin dosing with carbohydrate counting method on metabolic and clinical parameters in type 1 diabetes patients. MATERIAL AND METHODS: This study was conducted with patients following up at the Endocrinology Clinic with a diagnosis of type 1 diabetes mellitus between 2012 and 2015. Metabolic and clinical parameters before and after carbohydrate counting were compared. RESULTS: Forty patients were included in the study. Of the patients, 40% (n = 16) were female, and 60% (n = 24) were male, and mean age was 21.5 ± 7 year at the time of diagnosis. Statistically significant differences were not detected when haemoglobin A1c, fasting plasma glucose, post-prandial glucose, LDL-cholesterol, and HDL-cholesterol levels were compared at standard dose insulin use and after carbohydrate counting (P < 0.005). Among the parameters measured when the patients received standard dose of insulin without counting carbohydrate and flexible insulin dosing by counting carbohydrate, statistically, significant differences were not detected for baseline insulin dose, bolus insulin dose, triglyceride level, body mass index, or monthly hypoglycemia episodes (P > 0.05). CONCLUSION: Flexible insulin dosing with carbohydrate counting provides significant improvements in clinical and metabolic control. We detected improvements in lipid profiles and glycemic control. Additionally, patients generally did not gain weight despite flexible nutrition, and frequency of hypoglycemia remained unchanged despite strict glycemic control. © 2018 Feyzi Gokosmanoglu, Attila Onmez. The purpose of this study was to investigate the relationship between serumseladin-1 levels and clinical characteristics of patients with non-metastatic breastcancer, and to examine the prognostic value of seladin-1 in breast cancer.Methods: Patients aged 18 and over diagnosed with breast cancer usinghistopathological methods at our medical oncology clinic, whose tumor tissue hadbeen surgically removed and who had not yet received any oncological treatment, andwith no distant organ metastasis or additional malignancy, and healthy womenvolunteers as a control group were included in the study. Demographic and laboratorydata were recorded. Serum seladin-1 levels were compared between the patient andcontrol groups.Results: Seventy-three women, 46 patients and 27 controls, were enrolled. Mean ageswere 56±12 years in the patient group and 62±12 in the control group (p=0.055)Seladin-1 levels were lower in the patient group than in the control group (p=0.038).No statistically significant relationship was observed between tumor size and seladin1 levels (p=0.138). No relationship was also determined between patient grades andstages and seladin-1 (p=0.720; p=0.092, respectively).Conclusions: Seladin-1 levels were lower in the serum of breast cancer patients thanin the control group. However, no statistically significant relationship was foundbetween breast cancer prognostic factors and seladin-1 levels. Further research isneeded to clarify the mechanisms underlying the low seladin-1 levels in breast cancerpatients.Öğe Oftalmopatisiz Hipertiroidik Hastalarda Optik Cohorens Tomografi Bulguları(2023) Eski, Mehmet Tahir; Teberik, Kuddusi; Sezer, Taha; Önmez, AttilaAmaç: Bu çalışmada, hipertiroidi tanısı olan ve oftalmopati gelişmemiş hastalardaki koroid, retina ve peripapiller sinir lifi tabakasının kalınlığını ötiroid hasta grubuyla karşılaştırmak amaçlanmıştır. Gereç ve Yöntemler: Hastaların serum tiroid stimülan hormon (TSH), serbest T3 (fT3), serbest T4 (fT4) değerleri incelendi. Hipertiroidi semptomları ile başvuran ve tetkiklerinde TSH; 0,5 mu/L’nin altında saptanan olgular çalışmaya dahil edildi. Spektral domain optik koherens tomografi ile retina kalınlığı (RT), retina sinir lifi tabakası (RSLT) kalınlığı ve koroid kalınlığı (CT) hesaplandı. Bulgular: Çalışmaya 40’ı (%49) hipertiroidi hastası (HT), 42’si (%51) ötiroidik sağlıklı bireyler olmak üzere toplamda 82 kişi dahil edildi. RT ölçümleri açısından T500, T1000, T1500 ve N1500 değerlerinin HT grupta kontrol grubuna göre daha düşük olduğu olduğu ve bunların istatistiksel olarak anlamlı olduğu görüldü (sırasıyla pÖğe Predictivity of fatty liver index for non-alcoholic fatty liver disease in lean females with polycystic ovary syndrome(Makerere Univ, Coll Health Sciences,Sch Med, 2022) Arıkan, Didem; Önmez, Attila; Aksu, Erson; Tasdemir, NicelBackground: Fatty liver index (FLI) is a simple tool used to predict non-alcoholic fatty liver disease (NAFLD). The role of FLI in polycystic ovary syndrome (PCOS) for the prediction of NAFLD has not been elucidated. Methods: This case-control study was from January 2014 to January 2016. Anthropometric measurements, biochemical testing, and abdominal ultrasonography were performed in 83 premenopausal otherwise healthy women with PCOS and 58 controls. NAFLD was diagnosed by ultrasound. The predictivity of FLI for NAFLD in lean and overweight/obese females with PCOS was analyzed. Results: The ??-glutamyl transferase levels were significantly higher in the females with PCOS than in the controls (p = 0.001). In women with PCOS, FLI was significantly higher in females with NAFLD comparing to those without NAFLD (47.1 ?? 33.6 vs. 16.9 ?? 21.6; p = 0.001). For the PCOS group, Body Mass Index had the strongest relationship with FLI (p < 0.05, r = 0.908). FLI < 30 was calculated for all the lean females. The lean females with PCOS had a significantly higher rate of NAFLD (27.5% vs. 8.8%; p = 0.041) than lean controls. Conclusion: An FLI < 30 was not sufficient to rule out NAFLD in the lean PCOS patients.Öğe The Role of Meteorological Parameters in COVID-19 Infection(2020) Gülhan, Nevin, Pınar Yıldız; Balbay, Eda Güleç; Öztürk, C. Elif; Önmez, AttilaObjective: The SARS-CoV-2 infection outbreak was given the name CoronavirusDisease 2019 (COVID-19) by the World Health Organization. Meteorologicalparameters are one of the most important factors affecting infectious diseases. Theaim of this study is to analyze the correlation between meteorological parameters andthe COVID-19 pandemic.Methods: One hundred ninety-seven COVID-19 patients diagnosed and treated in theTurkish province of Duzce between 29.03.2020 and 04.05.2020 were included in thisstudy.Results: We found the relationship between air quality parameters and COVID-19case numbers revealed significant negative correlation between positive patientnumber and air temperature, relative humidity, and NO2, and significant positivecorrelation with air pressure, but no correlation with PM10, PM2.5, SO2, NO, or CO.Conclusions: Our findings are important as a preliminary study, since interactionsbetween air pollutants and meteorological factors may be involved in the transmissionand pathogenesis of COVID-19, and large-scale studies should now be designed for abetter understanding of these interactions.
