Şahiner; Kutlucan, LeylaObjectives: Providing proper nutritional support for the intensive care unit (ICU) patients is only possible with assessment of malnutrition. In this study, we aimed to evaluate associations between nutritional indexes which were developed to assess malnutrition using blood parameters, and survival among elderly patients in the ICU. Patients and methods: Patients older than 65 years of age and monitored in the ICU, were retrospectively screened with nutritional indexes in order to assess the nutritional status. Each patient was evaluated with both "Controlling Nutritional Status Index'' (CONUT) which includes albumin, total cholesterol, total lymphocyte counts, and "Prognostic Nutritional Index'' (PNI) which includes only albumin beside total lymphocyte count. Results: Two hundred twenty five patients were enrolled in this study. Median age was 79 (range: 65100). One hundred and eleven (49.3%) patients died during intensive care follow-up. CONUT and PNI scores displayed significant differences between patients who died and who were discharged (P = 0.02 and P = 0.03, respectively). Kaplan-Meier analysis revealed that high CONUT (>= 5) and low PNI (<40) scores were associated with intensive care unit survival but statistical significance was not provided for PNI (P = 0.04 and P = 0.06, respectively). Conclusion: Both CONUT and PNI indexes seems associated with survival but only CONUT score provides statistically significant prognostic information and may serve as a cost-effective nutritional assessment tool in elderly patients. Because this association did not retain its significance on multivariate analysis, the validity of this index in elderly patients should be further evaluated by prospective studies including larger samples. Yaşla birlikte eşlik eden hastalıkların artması bu hastaların takibinde ayrıca dikkat edilmesi gereken bir konudur. Biz de medikal yoğun bakım (YB) ünitemizde takip edilen yaşlı (65-79 yaş) ve çok yaşlı (80 yaş üstü) gruptaki hastalarımızın klinik ve laboratuvar özelliklerini karşılaştırdık.Yöntemler: 65 yaş ve üstü YB hastalarının klinik ve laboratuvar verileri retrospektif olarak tarandı. Hastaların komorbiditeleri 19 hastalık ağırlıklı Charlson Comorbidity Index (CCI) ile değerlendirildi.Bulgular: Çalışmaya 231 yaşlı ve 216 çok yaşlı hasta alındı. Her iki hasta grubunda da pulmoner hastalıklar YB'ye en sık primer alınış sebebiydi. Yine her iki grupta da hipertansiyon ve kalp yetmezliği en sık komorbid hastalıklardı. Çok yaşlı hasta grubunda CCI skoru 3 olanların sayısı daha fazla idi. Takip süresince gruplar arasında medyan YB-sağkalım açısından fark saptanmazken 30 gün ve üzerindeki YB takiplerinde ise median YB-sağkalım yaşlı hastalarda 81 gün (%95 CI: 48,8-113,2) iken çok yaşlı hastalarda 41 gün (%95 CI: 36,6-45,4) idi (p0,009).Sonuç: 65 yaşın üzerindeki hastalarda artan yaşın YBsağ kalımlarında primer faktör olmadığını saptadık. Ancak yoğun bakımda uzun süreli yatışlarda artan yaş kısa sağ kalımla ilişkili olabilir.Öğe Diagnosing Hematological Disorders Using Deep Learning Method(2021) Yurtay, Nilüfer; Öneç, Birgül; Karagül, TubaDeciding on the diagnosis of the disease is an important step for treating the patients. Also, the numerical value of blood tests, the personal information of patients, and most importantly, an expert opinion is necessary to diagnose a disease. With the development of technology, patient-related data are obtained both rapidly and in large sizes. Deep learning methods, which can produce meaningful results by processing the data in raw form, are beginning to give results that are close to human opinion nowadays. The present work is aimed to develop a system that will enable the diagnosis of anemia in general practice conditions due to the increasing number of patients and the intention of the hospitals, as well as the difficulties in reaching the expert medical consultant. The main contribution of this work is to make a diagnosis like a doctor with the data as the way the doctor uses it. The data set was obtained from the actual hospital environment and no intervention, such as increasing or decreasing the number of data, increasing or decreasing the number of attributes, reduction, integration, imputation, transformation, or discretization, has been made on the incoming patient data. It is related strongly with hypoxia. Hypoxic conditions increase mean platelet volume levels. This study aimed to investigate the effect of age on mean platelet volume in patients with marked nasal septal deviation. We made a retrospective study of patients with marked nasal septal deviation between January 2012 and May 2014. The patients were divided into four groups according to duration of nasal obstruction (less than 10, 10-20, 20-30 and more than 30 years). The groups were compared with each other in terms of mean platelet volume, platelet distribution width, platelet count in preoperative hemogram. This study was performed on 356 male and 139 female patients. Mean age was 33.9 +/- A 12.3 years. It was determined that the platelet count, mean platelet volume did not constitute statistically significant difference between groups (p > 0.05). Nevertheless, it was determined that as the duration of nasal obstruction elongated the mean platelet volume value increased and platelet count values decreased. Mean values of platelet distribution width constituted statistically significant difference between all groups (p = 0.026). This is especially important in critically ill elderly patients who have high mortality risk. This study aimed to investigate the effects of admission laboratory results and medical histories on the prediction of prognosis in critically ill elderly patients. Material and Methods: Patients who were >= 65 years and admitted to a medical intensive care unit (ICU) between 2011 and 2013 were retrospectively analyzed. Results: The study included 449 patients and mortality rate was 47.4%. Nonsurvivors had lower pH, HCO3 and albumin levels, and lower hematocrit and platelet counts, but higher aspartate aminotransferase, alanine aminotransferase, C-reactive protein (CRP), creatinine, phosphorus, magnesium and bilirubin levels, and higher leukocyte count than survivors. The rates of chronic kidney disease, being in a bedridden state and having cardiopulmonary resuscitation (CPR) before ICU admission were significantly high in nonsurvivors. Multivariate analysis showed that pH <7.20, albumin <= 2 gr/dL, low hematocrit and high CRP levels, high leukocyte count, bedridden state, and CPR were mortality predictors. After including the admission diagnoses and endotracheal intubation into the model, pH <7.20 (odds ratio [OR], 4.31; 95% confidence interval [CI], 1.59-11.70), albumin <= 2 gr/dL (OR, 3.61; 95% CI, 0.99-13.03), hematocrit level (OR, 0.94; 95% CI, 0.91-0.99) and leukocyte count (OR 1.06; 95% CI, 1.01-1.11) retained their prognostic importance for mortality. Conclusions: Severe acidemia, low albumin and hematocrit levels, and high leukocyte count at admission help clinicians to foresee the prognosis in severely ill elderly patients. Lung and kidneyinvolvement was observed. The clinical course progressedvery quickly, and catastrophic antiphospholipid syndromewas diagnosed with these findings. Two-thirds ofcatastrophic antiphospholipid syndrome cases developdue to secondary causes, the most common beinginfections.
