    Polidaktili sık rastlanan bir deformitedir. Herhangi bir sendromun parçası olabileceği gibi izole bir deformite şeklinde de karşımıza çıkabilir. En çok el, ayak ya da her ikisini birden etkileyebilen polidaktilinin sadece tek bir ayakta gözlenmesi oldukça nadir bir durumdur. Bu olguda, 1 yaşındaki bir erkek hastada gözlenen preaksiyel polidaktilinin çok nadir bir formu ve tedavisi sunulmaya çalışılmıştır. Hastanın inkomplet tarsal duplikasyonu ve aksesuar anterior tibialis tendonu mevcut olup ekstra parmakta fleksör ya da ekstansör tendon bulunmamakta idi. Ekstra parmak başarılı bir şekilde çıkarıldı ve aksesuar anterior tibial tendon medial küneiforma taşındı. Bu çalışmanın amacı sonuçlarımızı tartışarak nadir gözlenen bu polidaktili formuna dikkati çekmektir.

    Comparison of topical hypericum perforatum and metformin effectiveness in wound healing in streptozotocin-induced diabetic rats

    Purpose: Our study aimed to assess and compare the effectiveness of topically applied Hypericum perforatum (HP) and metformin on wound healing in diabetic rats. Methods: Thirty-two male Wistar rats (8-10 weeks old, 250 +/- 50g) were divided into four groups of eight rats each: Control, Diabetes mellitus (DM), Metformin (Met), and HP. Diabetes was induced in all experimental rats except the control group using streptozotocin (STZ) (60mg/kg, intraperitoneal). A full-thickness skin defect was created in all rats. Two milliliters (ml) of sterile saline were administered to the Control and DM groups, two ml of metformin (10% gel, topical) to the Met group, and two ml of HP (olive oil extract) to the HP group, repeated every 24 hours for 14 days. The condition of the lesions was monitored on days 0, 3, 7, 10, and 14, and the extent of contraction and granulation tissue formation was documented. At the 14(th) day, the lesioned areas were examined histopathologically and immunohistochemically. Results: The baseline characteristics of the rats before the study showed no significant differences between the groups (p>0.05). The HP group had the smallest final wound size and the highest wound contraction percentages from day 0 to 14 (p<0.001 for both). There was no statistically significant difference among the groups in the collagen index (p=0.118). The fibroblast density scores in the DM group were significantly lower than those in the other groups (p=0.004). The hypertrophic index values of the HP group were the highest compared to the other groups (p=0.003). Although the HP and control groups exhibited higher TGF-beta percentages and H scores than the other groups, these differences were not statistically significant (p=0.660 and p=0.647). Conclusion: Topically applied HP in uncontrolled DM rats improved wound healing scores more than the non-diabetic controls. Metformin also significantly enhanced healing in DM rats, with results comparable to controls. Since HP and metformin are easily accessible, further research could lead to cost-effective treatments for wound healing issues in DM patients.

    The effect of peripheral dopamine on fracture healing: an experimental study in a rat model

    BackgroundDopamine is a versatile biomolecule that functions as a neurotransmitter, hormone, and immune modulator in the body. Although some anabolic effects of dopamine on bone tissue have been described in the literature, its influence on the complex processes involved in fracture healing remains unclear. This study aimed to evaluate the effects of dopamine on bone healing at the peripheral level.MethodsThirty-six male Wistar albino rats were randomly assigned to two groups: a control group with no treatment and a dopamine group that received 12 mg/kg levodopa twice daily via oral gavage following surgery. A standardized femoral fracture was induced under anesthesia in all the rats, which were then fixed with an intramedullary Kirschner wire. Each group consisted of 18 rats, and six rats from each group were randomly sacrificed on postoperative days 15, 30, and 45. The harvested femurs were first evaluated radiologically, followed by biomechanical analysis via a three-point bending test, and finally subjected to histopathological examination.ResultsNo significant differences were observed between the groups on days 15 and 30. However, on day 45, histopathological scores were significantly lower in the dopamine group (p = 0.015), and biomechanical strength was also lower (p = 0.004). Radiological scores were not significantly different between the groups at any time point.ConclusionDespite the known anabolic effects of dopamine on bone cells, it may adversely affect fracture healing. The negative impact of dopamine on bone union could be attributed to the multifactorial and complex nature of fracture healing, the dynamics of inflammatory processes, and the cumulative effects of various dopamine receptor subtypes.

    Evaluation of the Reliability and Quality of YouTube Videos about Insulin Injection in Children

    Aim: Social media platforms, especially YouTube, have become indispensable channels where patients apply for information on medical issues. This study aimed to evaluate the reliability and quality of videos on YouTube about insulin injection in children.Material and Methods: The first 100 videos with the search “insulin injection in children” were analyzed. The distribution of video types, viewing rates, like ratios, number of comments, and video power indexes (VPI) were recorded. The content qualities were assessed by the global quality scale (GQS), modified DISCERN score, the Journal of the American Medical Association (JAMA) benchmark score, and the patient education materials assessment tool for audio/visual (PEMAT-A/V) materials.Results: Forty-seven (47%) videos were analyzed finally. The median score of the GQS was 4, modified DISCERN and JAMA was 3. The median PEMAT A/V understandability and actionability scores were 84 and 75.     Management preferences of orthopedic surgeons in rickets patients in Turkey: Results of a nationwide survey

    Aim Rickets remains a significant health issue, particularly in developing countries. This study evaluated the management preferences of orthopedic surgeons in Turkey regarding rickets and analyzed these approaches from a pediatric endocrinology perspective. Methods An online survey was developed based on a comprehensive literature review and previous similar studies. The survey link was distributed via email to members of the Turkish Orthopedic and Traumatology Association and the Pediatric Orthopedics Association. The questionnaire included 14 multiple-choice items addressing sociodemographic characteristics and the general approach to the diagnosis and treatment of rickets. Prior to dissemination, the survey was reviewed by 13 board-certified orthopedic surgeons from various institutions to ensure clarity and content validity. Results Of the 257 respondents, 198 met the inclusion criteria and were included in the analysis. Orthopedic surgeons with more than 10 years of experience (n = 111,56.1%) were significantly less likely to refer rickets patients to a pediatric endocrinologist (p = 0.009) and more likely to recommend oral or intramuscular 25-OH-vitamin D3 ampoule treatment rather than oral drops or capsules (p < 0.001). Surgeons who saw fewer than 25% pediatric patients in daily practice (n = 110,55.6%) were more inclined to refer patients without ordering diagnostic tests or initiating treatment (p = 0.011). The presence of a pediatric endocrinologist within the same institution (n = 96,48.5%) was also associated with increased referral rates without preliminary testing or treatment (p = 0.003/p = 0.002, respectively). Physicians who encountered at least one rickets patient per week (n = 78,39.4%) demonstrated better knowledge of normal serum 25-OH-vitamin D3 levels (p = 0.042) and were significantly less likely to refer patients to a pediatric endocrinologist (p < 0.001). Conclusion Management approaches to rickets among orthopedic and traumatology specialists in Turkey vary significantly depending on clinical experience, practice setting, and access to a pediatric endocrinologist. To prevent both undertreatment and unnecessary referrals, newly diagnosed rickets patients in orthopedic clinics should be appropriately evaluated and referred when necessary. Establishing a standardized diagnostic and referral algorithm based on international guidelines and incorporating it into orthopedic training programs is strongly recommended.

    Monosemptomatik Enürezis Nokturnalı Hastalarda Uyku Bozukluklarının Değerlendirilmesi

    Amaç: Bu çalışmada uyku bozuklukları ile monosemptomatik enürezis nokturna (MEN) arasındaki ilişkinin araştırılması ve MEN'e neden olan olası etiyolojik faktörlerin değerlendirilmesi amaçlandı. Gereç ve Yöntemler: Enürezis noktürna şikâyeti olan 50 hasta, Eylül 2012 ile Aralık 2013 tarihleri arasında ileriye dönük olarak değerlendirildi. Tüm olguların ayrıntılı incelemesi sonucunda ikincil organik nedenleri olan hastalar çalışma dışı bırakıldı. Ayrıntılı anamnez ve detaylı muayeneler neticesinde, primer MEN olduğu saptanan 34 hastaya (%68,0) bütün gece tam polisomnografi yöntemi ile uyku testi yapılarak kayıt altına alındı. Bulgular: Çalışmada değerlendirilen elli hastanın yirmi dokuzuna (%85,3) farklı derecelerde obstrüktif uyku apne sendromu tanısı konuldu ve bu hastalar çalışmaya dâhil edildi. Elde edilen apne skor değerlerinin; hastaların yaşı, alt ıslatma sıklığı, vücut kitle indeksi ve mesane kapasitesi ile korelasyonu analiz edildi ve analizler sonucunda sadece apne skoru değerleri ile vücut kitle indeksi arasında istatistiksel olarak anlamlı pozitif korelasyon tespit edildi (r=0,384; p=0,047). Hastalar ek olarak ağırlık gruplarına ayrılarak apne skoru, alt ıslatma sıklığı ve mesane kapasitesi açısından karşılaştırıldı ve alt ıslatma sıklığı ile mesane kapasitesi açısından anlamlı farklılıklar tespit edilmezken; obez grubun ortanca apne skoru normal ve fazla kilolu gruplardan istatistiksel olarak anlamlı derecede yüksek bulundu (p=0,019). Sonuç: Obezite ve obstrüktif uyku apne sendromu, MEN tanısı konan hastaların önemli bir kısmında mevcuttur ve bu iki hastalık, MEN gelişiminde etkili faktörlerdir.

    Monosemptomatik enürezis nokturnalı hastalarda uyku bozukluklarının değerlendirilmesi

    Amaç: Bu çalışmada monosemptomatik enürezis nokturna (MEN) şikayeti bulunan hastalarda uyku bozukluklarının araştırılması ve uyku bozukluğu ile MEN arasındaki ilişkinin değerlendirilmesi amaçlanmıştır. Gereç ve Yöntemler: Hastanemiz Çocuk Sağlığı ve Hastalıkları polikliniğine başvuran ve enürezis nokturna şikayeti bulunan 50 hasta alındı. 5 yaşın üzerinde, haftada en az 2 veya daha fazla alt ıslatması olan ve 3 aydan daha uzun süredir alt ıslatma problemi devam eden hastalar enürezis nokturna olarak kabul edildi. Vakaların hepsi enürezise neden olabilecek sekonder organik nedenler yönünden değerlendirildi. İki defadan fazla tekrarlayan idrar yolu enfeksiyonu geçiren, aşırı aktif mesane bulguları olan, enürezis diürnası olan ve üriner sistemle ilgili anomalisi olduğu tespit edilen hastalar çalışmadan çıkarıldı. Hastaların sorgulaması anket formu ile yapıldı. Hastaların öyküsü, özgeçmişi ve soy geçmişi alındı ve fizik muayeneleri yapıldı. Ayakta ve yatarak direk batın grafileri çekilerek üriner sistem ultrosonografileri yapıldı. USG ile mesane kapasitesi ölçümleri yapıldı. Anamnez ve tetkikler sonucunda primer monosemptomatik enürezis nokturna'sı olduğu tespit edilen olguların tümüne tüm gece full polisomnografi yöntemiyle uyku testi uygulandı. İstatistiksel analizler NCSS (Number Cruncher Statistical System) 2007 Statistical Software (Utah, USA) paket programı ile yapıldı. Sonuçlar, anlamlılık p < 0,05 düzeyinde değerlendirildi. Bulgular: Primer monosemptomatik enürezis nokturna (PMEN)'sı olan toplam 34 hastaya uyku testi uygulandı ve bu hastaların 29'unda (%85) değişik derecelerde uyku bozukluğu tespit edildi. PMEN hastalarının çoğunluğu erkek çocuklardan oluşmaktaydı. Tüm hastaların apne skoru ortalaması 4.99 ve alt ıslatma sıklığı ortalaması haftada 5 gün ve üzerinde (5.55) bulundu. Apne skoru sonuçlarına bakıldığında obstrüktif uyku apne sendromu (OSAS) derecesi; 20 hastada hafif (<5), 6 hastada orta (5-10) ve 3 hastada ağır (>10) olarak bulundu. Apne skoru değerleri ile BMI arasında pozitif yönde istatistiksel olarak anlamlı ilişki gözlendi (p=0.047). Obez grubun apne skoru ortalaması normal ve fazla kilolu gruptan istatistiksel olarak anlamlı derecede yüksek bulundu (p=0,042, p=0,048) ancak, normal ve fazla kilolu grupları arasında istatistiksel olarak anlamlı farklılık gözlenmedi (p=0,809). PMEN'lı ve OSAS (+) olan hastaların USG sonuçları incelendiğinde; 8 (%27.6) hastada mesane kapasitesinin yeterli ve 21 (%72.4) hastada da yetersiz olduğu tespit edildi. Mesane kapasitesi oranı değerleri ile alt ıslatma sıklığı arasında negatif yönde istatistiksel olarak anlamlı ilişki gözlendi (r=-0,441 p=0,017). Hastaların 14'ünde (%48.2) sık üst solunum yolu enfeksiyonu (ÜSYE) geçirme öyküsünün mevcut olduğu bulundu. Sık ÜSYE geçirme öyküsü olan ve olmayan grupların apne skoru ortalamaları karşılaştırıldığında istatistiksel olarak anlamlı bir farklılık saptanmadı (p>0.05). Sonuç: Çalışmamızda enürezis nokturna için başta gelen predispozan faktörler olarak pozitif aile öyküsü, obezite ve uyku bozukluğu tespit edildi. PMEN'nın tanımında sekonder nedenler dışlanmakla birlikte; çalışmamızda uyku bozukluğu ve obezitenin, enürezis etiyolojisinde kolaylaştırıcı ve belirleyici bir faktör olduğu düşünülmelidir. OSAS ile PMEN arasındaki ilişkiyi daha da açıklayan ileri araştırmalara ihtiyaç vardır.

    The association of sleep duration, wake-up time, and lifestyle habits with obesity in adolescents

    Objectives: Obesity is linked to excessive caloric intake, sleep behaviors, and the environment. Few studies explore the connection between wake-up time and obesity. This research aims to examine sleep patterns, timing, lifestyle, and sociodemographic factors associated with obesity among adolescents aged 15–17. Methods: This cross-sectional study at Ankara Bilkent City Hospital’s Pediatric Endocrinology Outpatient Clinic from December 2023 to November 2024 included 453 adolescents (217 normal weight, 236 obese) without comorbidities. Data on demographics, sleep patterns, physical activity, sedentary behavior, and diet were collected via face-to-face questionnaires. Obese participants were classified into Class 1 and Class 2–3 based on body mass index. Results: Obese participants woke up significantly later on weekdays (p=0.001). While normal-weight adolescents often woke between 7:00 and 8:00 AM, more obese adolescents woke after 9:00 AM. They also slept for more than 9 h more often (p=0.002). Obese adolescents spent more than 3 h in sedentary activity (p=0.022). Those with Class 2–3 obesity who did not exercise and had prolonged sedentary time were more common. Bedtimes did not differ significantly. Conclusions: A delayed morning wake-up time was significantly associated with obesity among adolescents. Sleep timing appears to be as relevant to metabolic health as sleep duration. Reduced physical activity coupled with increased sedentary behavior is associated with obesity. Therefore, advocating for improved sleep hygiene, fostering consistent physical activity routines, and implementing behavioral awareness training in adolescents should be regarded as essential elements of obesity prevention initiatives. © 2026 the author(s),

    The effects of liraglutide and metformin treatment on fracture healing in partially insulinopenic diabetic rats

    Purpose Although Metformin has been studied, the comparative or synergistic effect with GLP-1 agonists like Liraglutide on fracture healing remains poorly characterized. This study aimed to evaluate the impact of metformin, liraglutide, and their combination on fracture healing in a rat model of partially insulinopenic diabetes mellitus (DM). Methods Sixty male Wistar rats (10-14 weeks old, 350 +/- 50g) were divided into five groups of twelve rats each: Control, DM, Met (Metformin), L (Liraglutide), and Met+L. Partially insulinopenic DM was induced in all experimental rats except the control group using streptozotocin (STZ) and nicotinamide (NA) combination. A femoral fracture was created, and a Kirschner wire was inserted retrogradely into the femoral canal. Liraglutide was injected subcutaneously at a daily dose of 0.6 mg/kg into the rats in the L and Met+L groups, and oral metformin was administered to the rats in the Met and Met+L groups daily at a dose of 180 mg/kg. On the 15th, 30th, and 45th days, four rats from each group were selected randomly and euthanized, and the femurs were examined radiographically, biomechanically, and histopathologically. Results The baseline characteristics of the rats before the study showed no significant differences between the groups (p>0.05). Biomechanical test results showed a significant main effect of group (p<0.001), indicating that overall Newton values varied across groups. Additionally, a significant main effect of experimental day was found (p<0.001), suggesting that Newton values changed across days regardless of group. Histopathological scores showed a statistically significant difference between the groups on the 15th day, with the L group having 75% scoring 7 (p=0.047), and on the 45th day, with the L and Met+L groups both having 75% scoring 9 (p=0.036). Conversely, no significant difference was found in radiological scores between the groups on the 15th day (p=0.934), 30th day (p=0.649), and 45th day (p=0.502) of the experiment. Conclusion Both metformin and liraglutide improve fracture healing in a partially insulinopenic diabetic rat model, and these findings suggest that liraglutide may offer a superior therapeutic advantage over metformin in accelerating fracture repair in patients with diabetes.

    An Unusual Preaxial Polydactyly of the Foot; a Case Report

    Polydactyly is a commonly seen disorder. It can be seen as part of a syndrome or isolated deformity. Most commonlyaffects hand, foot or both but the presence of extra digit in only one foot is a very rare condition. We here present a veryrare case of preaxial polydactyly of the foot with incomplete tarsal duplication, an accessory anterior tibialis tendon andwithout any flexor or extensor tendons in a 1-year-old male. The extra digit was removed successfully and the accessoryanterior tibial tendon was repaired to the medial cuneiform. The purpose of this report was to discuss our results and totake attention to this rare presentation of polydactyly.
