    Antibiotic Resistance Rates of Pseudomonas Aeruginosa Strains Isolated from Clinical Samples

    Purpose: Pseudomonas aeruginosa species are common bacteria that cause nosocomial infections. In this study, we aimed to contribute to epidemiological data and empirical treatment options by determining the resistance rates of P. aeruginosa strains isolated from clinical samples sent to our laboratory. Materials and Methods: Conventional microbiological methods and Vitek 2 (bioMérieux, France) automated system were used for the identification and antibiogram processes of P. aeruginosa strains isolated from various clinical samples sent to our laboratory between November 2016 and August 2018. Results: Seventy-two (29 %) deep tracheal aspirate (DTA), 68 (27 %) urine, 50 (20 %) sputum, 24 (10 %) bronchoalveolar lavage (BAL), 22 (9 %) wounds and 14 (5 %) from other samples (abscess, blood culture, tissue culture, pleura, catheter), a total of 250 strains were examined. The antibiotic that has the lowest resistance was found to be amikacin. Ciprofloxasin(50 %) and ceftazidime(48 %) had the highest resistance. Ciprofloxacin, ceftazidime, piperacillin-tazobactam (TZP) and gentamicin resistance were found to be higher in the intensive care unit than other clinics. It was found that the imipenem resistance rate in the samples from the polyclinics was very low (15 %) compared to the service and intensive care units. Conclusion: It was observed that the rate of resistance to antibiotics, which are frequently used in P. aeruginosa treatments in our hospital, has gradually increased over the years. It was observed that the resistance rate of cefepime, which had a high rate of resistance before, decreased. The high resistance rates we found in Pseudomonas strains show the importance of regulation of treatment according to antibiotic susceptibility tests.

    Düzce İlinde Son Bir Yılda Görülen Yurtdışı Kaynaklı Dört Sıtma Olgusu

    Sıtma,dünyada tropikal ve subtropikal bölgelerde endemik olarak görülmektedir. Sonyıllarda seyahatler ve göçler nedeniyle endemik olmayan bölgelerde de dışkaynaklı sıtma olgularıyla karşılaşılmaktadır. Bu olgu sunumunda ilimizde sonbir yılda görülen yurt dışı kaynaklı dört olgu irdelenmiştir. Olgularınhepsinin Afrika’ya seyahat öyküsü olup, ikisinin sıtma profilaksisi almadığıöğrenilmiştir. Tüm olgularda tanı; ince yayma ve kalın damla preparatlarınınincelemelerinde plazmodyum görülmesi ile konulmuştur. İki olguda yapılan PCRtestinin sadece birinde pozitiflik saptanmıştır. Etken, iki olguda Plasmodium falciparum, diğer iki olgudaise Plasmodium vivax olarakbelirlenmiştir. P. falciparum sıtmasıolan bir olgu derin anemi nedeniyle yoğun bakımda takip edilmiş ve tüm olgularşifa ile taburcu edilmiştir. Bu olgu sunumunda endemik olmayan bir bölgedegörülmüş olan yurt dışı kaynaklı sıtma vakalarının özellikleri toplu olarakincelenmiştir. Hastanelere ateş nedeniyle başvuran hastalarda sıtmanın endemikolduğu bölgelere seyahat sorgulanmalı ve anamnezinde seyahat öyküsü olan ateşlihastalarda ayırıcı tanıda sıtma düşünülmelidir.
