Because of this, by adjusting corrosion-resistant surfaces into total prosthesis, surface changinghip arthroplasty which has a great advantage protecting proximal femoral bone stocks has become a current issue. Inthis case report, we define a 45-year-old patient who has bilateral grade 2-3 hip avascular necrosisis, had haddecompression and bone marrow derivation but still has complaints, and who had bilateral total hip surface changingarthroplasty because of progressive necrosis.Öğe Ön-Alt İliak Çıkıntının Alışılmadık Şekildeki Kopma Kırığı: Nadir Bir Olgu(Düzce Üniversitesi, 2013) Karaduman, Zekeriya; Solak, Kazım; Arıcan, MehmetApofizyal spina iliaka anterior inferior kırıkları nadir yaralanmalardır. Bu yaralanmalar en ccedil;ok ad ouml;lesan d ouml;nemdeki sporcularda meydana gelir. Pelvisin ossifikasyonu tamamlanmadığından apofizler bu d ouml;nemde muskulotendin ouml;z birimin en zayıf par ccedil;alarıdır, bu nedenle av uuml;lsiyon kırıkları kas r uuml;pt uuml;rlerinden daha sık g ouml;r uuml;l uuml;r. Kırık rektus femoris kasının d uuml;z başının ccedil;ekmesi sonucu oluşur. Hastalar kasık b ouml;lgesinde şiddetli bir ağrı hissederler ve y uuml;r uuml;yemezler. Yatak istirahati, analjezi, koltuk değneği kullanımı ile başlanarak parmak ucunda y uuml;r uuml;me ve tolere edilebildiği ouml;l ccedil; uuml;de tam y uuml;k vererek y uuml;r uuml;meye kadar uzanan konservatif tedavi y ouml;ntemleri ile iyi sonu ccedil;lar elde edilebilmektedir. Bizim olgumuzda 17 yaşındaki erkek hastada nadir g ouml;r uuml;len bir yolla oluşmuş spina iliaka anterior inferior kırığını sunmayı ama ccedil;ladık.Öğe Ratlarda Oluşturulan Deneysel Diz Osteoartrit Modelinde İntraartiküler Glukozamin Sülfat Uygulamasının Kondroprotektif Etkisi(Düzce Üniversitesi, 2013) Karaduman, Zekeriya; Yücel, İstemi; Solak, KazımThe aim of this study is to examine the condroprotective efficacies of intraarticular glucosamine-sulfate administrationsin an experimental osteoarthritis model in rats. There are 2 groups in this study. 15 rats for each group (total 30) wereused in this study. The knee joints of the rats were seen macroscopically and the anterior cruciate ligaments (acl) wereresected. Two weeks after the surgery, three injections were administered intra-articularly once two weeks. 12mg/0.06cc glucosamine-sulfate was injected to the intraarticular cavity to the rats in group 1 (glucosamine-sulphate group). 50ul/0.05 cc 0.9% NaCl solution was injected to the intraarticular cavity to the rats in group 2 (control group). The ratswere sacrificed eight weeks later and the preparations were evaluated histopatologically. In the osteoarthritic rat kneejoints, the histopathological effects of tissue regeneration of glucosamine sulfate and HA applications compared to thecontrol group were shown to be statistically significant. Glucosamine sulphate administration can be accepted as analternative treatment protocol in the treatment of osteoartritis in the knee joint.Öğe Surgically Treated Posterior Acetabular Fractures Via Iselin’s Modified Approach with A Short-Term Follow-Up(Ordu Üniversitesi, 2019) Değirmenci, Erdem; Orhan, Zafer; Arıcan, Mehmet; Karaduman, Zekeriya; Turhan, Yalçın; Turhal, OzanObjective: The main purpose of the surgical treatment of posterior acetabular fracture is to achieve anatomical reduction to attain a functional and stable hip joint without pain. Although Kocher-Langenbeck (K-L) approach is the most commonly used surgical exposure, various modified approaches have been described in the literature. The aim of this study to determine the early surgical results of the acetabulum posterior fractures surgery via Iselin's modified K-L approach. Methods: We reviewed the hospital records of patients who were operated for acetabulum posterior wall fractures via Iselin's modified approach between 2016 and 2018. All patients had detailed radiological, clinical evaluation and fractures were classified by AO/ASIF classification. All patients had radiological and clinical evaluation at the end of the postoperative 1st year. Results: There were 16 men and 4 women with an average age of 42.8±18.0 (range 18-77) years. The average follow-up was 14.8±6.1 (range 6-28) months. The right hip was involved in 12 (60%) patients and the left in 8 (40%) patients. The average operative time was 78.6±16.7 (range, 54–115) minutes. Average blood loss during the operation was 179.22±51.9 (range, 100–260) ml. The postoperative reduction was graded as anatomic (0–1 mm of displacement) for 17 hips and imperfect (2–3 mm of displacement) for 3 patients. No patient had a deep infection, implant loosening, recurrent dislocation, deep venous thrombosis (DVT), pulmonary embolism (PE) or revision fixation. Conclusion: The modified approach of Iselin is a successful approach with its ease in the surgery of displaced fractures that extending proximally and early radiological and functional results.Öğe Tüberküloz Trokanterik Bursit: Bir Olgu Sunumu(Düzce Üniversitesi, 2015) Arıcan, Mehmet; Orhan, Zafer; Erbil, Barış; Karaduman, Zekeriya; Oktaş, BirhanTuberculosis trochanteric bursitis accounts for around only %1 of all cases of musculoskeletal tuberculosis andmusculoskeletal tuberculosis is about %10 of extrapulmonary tuberculosis. Tuberculosis trochanteric bursitis associatedwith chronic nonspecific findings, such as local tenderness, swelling, pain. Differential diagnosis is difficult because ofnonspecific findings in plain radiographs and computed tomography. Inflammatory findings are determined in magneticresonance and scintigraphy. Definitive diagnosis is confirmed with biopsy, acid-fast staining and culture. Rice bodiescan see. Lymphocytes, histiocytes, plasma cells, langerhans cells with caseous necrosis in granulomatous inflammatorytissue are presented in histopathology. Surgical resection, debridement, irrigation and anti-tuberculosis drugs use intreatment. A 30 years old female patient was treated surgically due to trochanteric bursitis in another clinic last year.The patient was admitted to our clinic due to recurrence of the symptoms. Rice bodies had seen in surgery period in ourclinic. The patient received antituberculosis therapy for 10 months, after which complete remission was obtained
