Öğe Evaluation of Thyroid Ultrasound and Thyroid Hormone Levels in Obese Patients who applied to the Outpatient Clinic of Family Medicine(Duzce Univ, 2016) Yılmaz, Aylin; Kara, İsmail Hamdi; Baltacı, Davut; Çeler, Hesna; Hakan, Leziz; Güçlü, Derya; Memişoğulları, RamazanObjective: In the present study, we aimed to evaluate obesity in terms of its effect on thyroid function and morphology. Methods: We included 327 consecutive obese patients (mean age 41.0 +/- 11.6) who applied to the Department of Family Medicine Obesity and Check-up Polyclinic of Dzce University School of Medicine and had an ultrasound scan (USG) between the months January to July 2012. Cases were classified as patients with obesity (n=130), patients with Type 2 Diabetes mellitus (DM) (n=27) and patients with Metabolic Syndrome (METS) (n=170). USG findings, as well as hematological, biochemical and hormonal parameters, were compared among these groups Results: When body mass index (BMI) values were compared between the groups, the highest results were found in DM group, and these results were statistically significant. In USG examinations of patients, 108 (29.2%) patients had nodules. There was no difference between the groups in terms nodule prevalence. When thyroid volumes were compared between the groups, the largest volumes were found in the DM group; however, this was statistically not significant. When thyroid stimulating hormone (TSH) levels were compared, the highest levels were in the obesity group; however, these results were again statistically insignificant. We observed that obese women and women with METS had higher TSH levels. Thyroid disorders were found less frequent in the DM group than the other groups, and these results were statistically significant. Conclusion: Although no significant difference was found between the groups in our study in terms of thyroid pathology, in the light of other studies, the prevalence of thyroid dysfunction in diabetic patients and patients with METS is 2-3 times higher than the normal population. Therefore the measurement of TSH should be made for all obese patients.Öğe Investigation of Demographic Features and Symptoms at Admission of Patients whose Apply to a Family Health Center in City Center of Düzce: Preliminary Study(Düzce Üniversitesi, 2015) Türker, Yasemin; Hakan, Leziz; Baltacı, Davut; Dikici, Bünyamin; Kara, İsmail HamdiAim: The aim of present study is investigating that demographic features and symptoms ofpatients at admission to a family health center (FHC) in city center of Düzce and is determiningthat important of the most common symptoms for training of family medicine specialty.Method: This cross-sectional and descriptive study is depending on medical data. In presentstudy, it was investigated that demographic features (age, gender, marital status, have or havenot children, social insurance status and insurance type) smoking status, most common firstfive symptoms or findings at admission of patients, chronic disease and status of medical drugprescription in a FHC in city center of Düzce.Results: Total 330 consecutive patients who are applied to a FHC in city center of Düzce wereinvestigated during 1-31 December 2013. While 176 of (53,3%) patients were female and 154of patients (46,7%) were male; 8,5% of females and 27,3%, of males, totally 17,3% of patientswere current smoker. Most common applicant to FHC are 45-65 aged adults and 18-44 agedyoung adults. Most of patients were applied for prescribing their medicine (155 cases %47,0).Of the patients, 94 cases (28,5%) had hypertension, 39 cases (11,8%) diabetes mellitus, 12 cases(3,6%) had hyperlipidemia and seven cases (2,1%) had rheumatologic disease, consecutively.When were symptoms of patients examined, the most frequent five symptoms were as: sorethroat, cough, fever, fatigue and headache, consecutively.Conclusion: Herein, it was showed that there are differences between in the current status ofprimary care patients and the causes of application to healthcare settings after Turkish HealthcareReform and conditions during previous heath care providing.
