Öğe A Case of Hantavirus Renal Syndrome Detected in the COVID-19 Pandemic(Duzce Univ, Fac Medicine, 2022) Cakir, Yasemin; Ince, NevinHantaviruses are enveloped RNA viruses in the Bunyaviridae family that cause rodent -borne zoonotic infections. They cause two separate diseases, hemorrhagic fever with renal syndrome (HFRS) and hantavirus cardiopulmonary syndrome (HCPS), as a result of transmission to humans through contact with rodent excrements and inhalation. The form seen more common in T & uuml;rkiye is HFRS, which progresses with acute kidney injury and thrombocytopenia. Coronavirus disease 2019 (COVID-19) is an infectious disease that ranges from asymptomatic infection to pneumonia, respiratory failure, and death. Because of symptoms such as fever, weakness, and flu -like clinical findings in the early days, it can be confused with many infectious diseases. In this case report, a case of hantavirus renal syndrome admitted with fever, weakness, and flu -like symptoms during the COVID-19 pandemic was presented.Öğe AN EVALUATION OF PATIENTS WITH TULAREMIA IN DUZCE PROVINCE IN THE WESTERN BLACK SEA REGION OF TURKEY(Parlar Scientific Publications (P S P), 2020) Ince, Nevin; Cetinkaya, Ozlem; Tunca, Bekir; Cakir, YaseminTularemia is a zoonotic infectious disease caused by Francisella tularensis. The aim of our study was to examine the epidemiological and demographical characteristics, laboratory findings, treatment methods, and treatment results of 68 tularaemia cases reported from Duzce, Western Black Sea region of Turkey, between 2016-2019. Clinical diagnosis was confirmed by micro-agglutination test (>= 1/160 titres). A total of 68 patients (35 females, 33 males) were diagnosed with tularemia. The most common symptoms among the patients in the study were swelling in the neck in 88.2% patients, sore throat in 73.5% patients and tonsillopharyngitis in 72.8% of the patients. Lymph node surgery was performed on 29.4% of patients despite receipt of antibiotherapy. The rate of ciprofloxacin use among patients not requiring lymph node surgery(46%) was significantly higher than that in patients requiring such surgery (20%) (p=0.046). No difference in lymph node surgery was observed among patients receiving combination antibiotherapy (p>0.05).Öğe Frequency of Hepatosteatosis and Relationship Between Laboratory Parameters and Hepatosteatosis in Chronic Hepatitis B Patients(Galenos Publ House, 2024) Cakir, Yasemin; Sungur, Mehmet Ali; Oez, Murtaza; Bueyuektuna, Seyit AliObjectives: Hepatosteatosis is very common worldwide and is defined as the accumulation of lipid droplets in hepatocytes. Hepatosteatosis is often associated with metabolic factors such as obesity, insulin resistance, and hypertriglyceridemia. The relationship between chronic hepatitis B (CHB) and hepatosteatosis remains unknown. We investigated the frequency of hepatosteatosis in patients with CHB and to evaluate the relationship between hepatosteatosis and laboratory parameters. Materials and Methods: We retrospectively studied 262 patients with CHB. Patients were divided into two groups, hepatosteatosis and non-hepatosteatosis, according to liver ultrasonography findings. The groups were compared in terms of demographic characteristics and laboratory parameters. Results: A total of 262 patients with CHB were included. The mean age was 45.1 +/- 15.3 years, 136 (51.8%) of whom were male. Liver biopsy was performed in 86 (32.8%) of the patients, 20 (7.6%) had fibrosis, and 163 (62.2%) had steatosis. Among the patients with steatosis, grade 1 steatosis was observed in 30.9 (81/163), grade 2 in 26 (68/163), and grade 3 in 5.3 (14/163). Hypertension, hepatomegaly, and cirrhosis were correlated with the presence of steatosis. Patients with steatosis were older than those without. Fasting glucose levels, low-density lipoprotein levels, and triglyceride levels of patients with steatosis were higher than those of patients without steatosis. High-density lipoprotein levels were lower in the steatosis group. No correlation has been found with gender, body mass index, hepatitis delta virus co -infection, hepatitis B virus (HBV)-DNA levels, or hepatitis B e antigen status between steatosis. Conclusion: We found that hepatosteatosis is present in a significant proportion of patients with CHB. Although the presence of hepatosteatosis was associated with some metabolic parameters, the relationship between it and HBV parameters was not statistically significant.Öğe A Rare Manifestation of Leptospirosis: Long-Term Elevation in Liver Enzymes(Duzce Univ, Fac Medicine, 2023) Cakir, Yasemin; Yilmaz Gokce, Ozlem; Tunca, BekirLeptospirosis is a bacterial zoonosis that is endemic in many tropical and subtropical regions. The disease is transmitted to humans by contact with the urine or an environment contaminated with the urine of infected animals. The clinical manifestations of leptospirosis vary from subclinical infection to severe illness with multiorgan dysfunction. This case report aimed to present a leptospirosis case diagnosed with detailed anamnesis and progressed with long-term elevation in liver function tests, in a non-endemic region. A 28-year-old male patient was admitted with complaints of weakness, fever, and vomiting. As a result of the detailed anamnesis, it was learned that the patient with hyperbilirubinemia, elevated liver enzymes, and kidney failure had contact with polluted water in a rural area, and the patient was followed up with a preliminary diagnosis of leptospirosis. Leptospirosis may present with different clinical presentations and be confused with many diseases, risk factors should be carefully questioned.
