Öğe Aleksitimi, Ruminasyon ve Stresle Başa Çıkmanın Psöriyazis ile İlişkisi ve Psöriyazis Kliniğine Etkisi(2020) Baysak, Sevim; Kılıç, Fatma Arzu; Karagün, Ebru; Baysak, ErkanAmaç: Psöriyazis hastalığında, psöriyatik plaklarda artış ile stres arasındailişki olduğu gösterilmiştir. Psöriyazis, ayrıca yüksek aleksitimi düzeylerive stres ile de ilişkilidir. Depresyon ve çeşitli psikiyatrik hastalıklarda,ruminasyon ile hem hastalığın başlaması hem de süreğenleşmesiarasında ilişki olduğu bildirilmiştir. Bu çalışmada stresle ilişkili diğer birfaktör olan ruminasyon incelenmiştir.Yöntem: 96 psöriyazis hastası ve 93 sağlıklı kontrol çalışmaya alınmıştır.Katılımcıların tamamına Sosyodemografik Veri Formu, TorontoAleksitimi Ölçeği, Stresle Başa Çıkma Tarzları Ölçeği, RuminatifDüşünme Biçimi Ölçeği ve Hastane Anksiyete Depresyon Ölçeğiuygulanmıştır. Hasta grubunda, psöriyatik plaklarda son 1 ayda artışolup olmadığı klinisyen tarafından kayıt altına alınmıştır.Bulgular: Hem hasta hem de kontrol grubunda ruminasyon puanlarıile aleksitimi puanları (hasta grubu, r=0,47; p<0,01; kontrol grubu,r=0,38; p<0,01) ve çaresiz stresle başa çıkma tarzı puanları (hasta grubu,r=0,56; p<0,01; kontrol grubu, r=0,57; p<0,01) arasında anlamlı ilişkisaptanmıştır. Depresyon ve anksiyete puanları kontrol edildiğinde hastagrubunda, ruminasyon puanları ile duyguları tanıma güçlüğü puanları(r=0,42; p<0,01), duyguları ifade güçlüğü puanları (r=0,25; p<0,05) veçaresiz stresle başa çıkma becerileri puanları (r=0,41; p<0,01) arasındapozitif yönde anlamlı ilişki olduğu görülmüştür. Depresyon ve anksiyetepuanları kontrol edildiğinde kontrol grubunda ise ruminasyon düzeyiile duyguları tanıma güçlüğü puanları (r=0,27; p<0,05) ve çaresiz streslebaşa çıkma becerileri puanları (r=0,42; p<0,01) arasında pozitif yöndeanlamlı ilişki olduğu görülmüştür. Psöriyatik plaklarda son 1 ayda artışolan ve olmayan hastalar karşılaştırıldığında, ruminasyon puanları(p<0,01), duyguları ifade etmede güçlük puanları (p<0,05) ve toplamaleksitimi puanları (p<0,05) arasında anlamlı fark olduğu saptanmıştır.Sonuç: Psöriyaziste aleksitimi ve pasif stresle başa çıkma tarzlarınınruminasyon ile ilişkisi hastalık seyrini etkiliyor olabilir.Öğe Demographic findings of patients diagnosed with pernio and comparison of their vitamin B12, folate and ferritin levels with a control group(2019) Karagün, Ebru; Baysak, SevimBackground and Design: Pernio is an inflammatory disease that occurs in the acral regions of the body after exposure to cold. The studyaimed to investigate the demographic findings of patients diagnosed with pernio and to determine the role of vitamin B12, folate and ferritinvalues in the pathogenesis of pernio.Materials and Methods: This study was conducted in pernio patients diagnosed at the Ağrı State Hospital, Dermatology Outpatient Clinic(Ağrı, Turkey) between September 2015 and April 2016. The healthy control group participants were of similar age and gender. Vitamin B12,folate and ferritin levels were compared between the groups.Results: The patient and control groups were similar in age and gender. Vitamin B12 and ferritin levels were significantly lower in the patientgroup (p<0.05). Folate levels did not differ significantly between the groups (p>0.05).Conclusion: According to the data obtained from our study, when the demographic findings of the pernio patients were compared with thosein the literature, the results were found to be similar. In the patients with pernio, it is likely that low Vitamin B12 was involved in the pernioetiology via the mechanism of vasoconstriction/vasodilatationÖğe Levels of TNF-alpha, IL-6, IL-17, IL-37 cytokines in patients with active vitiligo(Taylor & Francis Ltd, 2021) Karagun, Ebru; Baysak, SevimObjectives Vitiligo is an autoimmune disease, and its pathogenesis involves changes in cytokine levels in the affected patients. Tumor necrosis factor-alpha (TNF-alpha), interleukin (IL)-6, and IL-17 from pro-inflammatory cytokines, IL-37 in a recently detected anti-inflammatory activity. The aim of our study was to determine serum TNF-alpha, IL-6, IL-17, IL-37 levels in patients with vitiligo to understand their possible roles in the disease etiology and to compare the results with the healthy controls. Methods The study included 48 generalized vitiligo patients who were diagnosed with vitiligo, had an increase in the lesions within the last 3 months, and did not receive any systemic or topical treatment during this period; furthermore, 18 healthy controls were included. Results Patient group: n = 48, mean age = 30.48 +/- 9.86 years; control group: n = 18, mean age = 28.27 +/- 9.66 years. Individuals in the patient group had significantly higher serum levels of IL-37(t = 3.90, p < .001), IL-6 (t = 3.39, p < .05), IL-17 (t = 2.08, p < .05), and TNF-alpha (t = 4.69 p < .001) than in the control group. Conclusion The high levels of (pro-anti) inflammatory cytokines in vitiligo patients draw attention to the importance of cytokines in the pathogenesis of the disease.Öğe Novel mutation identified in the DDB2 gene in patients with xeroderma pigmentosum group-E(Wiley, 2020) Karagun, Ebru; Eroz, Recep; Gamsizkan, Mehmet; Baysak, Sevim; Eyup, Yavuz; Ozcan, YunusBackground Xeroderma pigmentosum (XP) is a rare photosensitive syndrome, which is divided into eight complementation groups (XP-A to XP-G and XPV) and characterized by skin cancers diagnosed at early age. A family of seven members (age range between 5 and 47 years) with carriers of the novel nonsense mutation that causes XP-E type were included in the current study. Methods DNA was isolated from peripheral blood samples of the proband, and cancer predisposition genes were sequenced with next-generation sequencing. The demographic features and the laboratory, clinical, and histopathological findings of patients were evaluated. Results In the proband, squamous cell carcinoma was first diagnosed in the right-eye cornea at the age of 13 years and then in the left-eye cornea at the age of 15 years. Later, the patient was diagnosed with basosquamous cell carcinoma on the dorsum of the nose at the age of 18 years. After genetic analysis, a novel nonsense c.1063C>T(p.Arg355Ter) pathogenic variation that causes XP-E type was detected as homozygous in the DDB2 gene of the proband and her siblings, 11 and 5 years of age, and as heterozygous in her parents and a 22-year-old brother. Conclusion Because of the occurrence of early termination codon, truncated nonfunctional proteins or proteins with deleterious loss or gain-of-function activities are synthesized in nonsense mutation. Thus, to avoid the development of pathological lesions, it is important that such patients with nonsense mutation stay away from agents that might cause DNA damage and develop an appropriate lifestyle according to this condition.
