    A Rare Case: Schilder Disease with Symmetrical Involvement

    Schilder disease or myelinoclastic diffuse sclerosis is rare disease which is characterized byinflammatory demiyelinization plaques in white matter. This disease is often mistaken foradrenoleucodistrophy and intracranial tumor. A-thirteen-year old patient who applied with thecomplaints of sudden onset of walking and speech loss and visual loss was thought to besuffering from Schilder and adrenoleucodistrophy by magnetic resonance image appearancebut adrenoleucodistrophy was excluded with laboratory findings. We present this due to itssymmetrical involvement and discussed the case with radiologic findings

    A Rare Combination: Myocardial Infarction and Cerebral Ischemia

    Myocardial infarction (MI) is death of heart cells due to the decreased blood flow to a part ofthe heart. Reduction in blood flow increases the oxygen demand of the heart and if it leftuntreated for a sufficient period of time, can cause damage or death of heart muscle tissue.Cerebral ischemia,represents metabolic damage in the brain as a result of insufficient bloodflow. Cerebral hypoxia leads to death of brain tissue that named cerebral infarction. Symptomsof brain ischemia can include unconsciousness (from somnolance to stupor), blindness,coordination problems, body weakness, cardiorespiratory arrest and irreversible brain damage.Observation of cerebral ischemia and MI are a rare combination. Combination of MI andcerebral ischemia is a rare situation we evaluated the case in light of recent literature

    Abdominal pain due to hereditary angioedema: Case Report [Herediter anjiyoödeme ba?li karin a?risi]

    Hereditary angioedema is a disease characterized by subcutaneous edema without urticaria, upper airway obstruction, and gastrointestinal symptoms. It results from congenital deficiency of C1 esterase inhibitor. This rarely seen disease is autosomal dominant inherited, and severe forms can be life threatening. Here, we present a case of hereditary angioedema who applied to the emergency room with severe abdominal pain, with low levels of C1 esterase inhibitor and C4 and with specific ultrasound findings, and completely recovered with the application of C1 esterase inhibitor concentrate.     Persistan median arter (PMA), elin vasküler yapısının anatomik bir varyasyonudur ve genellikle asemptomatiktir. Persistan median arterin varlığı ve trombozu, başta karpal tünel sendromu olmak üzere çeşitli komplikasyonlara yol açabilmektedir. Akut PMA trombozunun erken tanı ve tedavisi önemlidir; zira erken antikoagülan tedavi ile birçok komplikasyon ve cerrahi gereksinimi önlenebilmektedir. Bu yazıda, 36 yaşında bir kadın olguda akut el bileği ağrısına neden olan tromboze PMA'nın görüntüleme bulguları sunuldu. Erken antikoagülan tedavi, semptomların tamamen iyileşmesini sağladı. Hastada karpal tünel sendromu veya başka bir bulgusu yoktu

    ASSESSMENT OF CAROTID INTIMA-MEDIA THICKNESS IN ANKYLOSING SPONDYLITIS PATIENTS

    Background and aim: Measurement of the intima media thickness (IMT) of the common carotid artery has been a useful index to identify premature atherosclerosis. In the present study, we aimed to compare the carotid IMT of ankylosing spondylitis (AS) patients with that of controls matched for age and sex. We also evaluated the risk factors for atherosclerosis related to AS. Patients and methods: Fifty-five patients with AS and 55 healthy controls were included in the study. All of the patients underwent a clinical evaluation that included the following: Bath AS Disease Activity Index (BASDAI), Bath AS Functional Index (BASFI). Bath AS Patient Global Score (BAS-G), chest expansion, and modified Schober test. The same radiologist performed IMT measurements. Results: The mean carotid IMT was 0.59 +/- 0.15 mm in patients and 0.50 +/- 0.10 nun in controls (p=0.001). Disease duration, Schober index, and chest expansion were correlated with risk of atherosclerosis in the patients with AS (r=0.301, r=-0.369, and r=0.511, respectively). Otherwise, BASDAI, BASFI, BAS-G, ESR, hsCRP and treatment seemed to have no effect on CIMT. Conclusion: Our study shows that CIMT was significantly greater in the AS patients than in the controls paired for age and sex.

    Breast arterial calcifications and carotid intima-media thickness and haemodynamics: Is there any association?

    Objective: In this study, we aimed to research the relation between breast arterial calcifications (BACs) detected on mammography and two well-known markers of cardiovascular diseases-carotid artery intima-media thickness (C-IMT) and haemodynamics parameters like carotid peak-systolic velocity (PSV), end-diastolic velocity (EDV) and resistive index (RI). Methods: The study group consisted of 50 consecutive BAC (+) women and the control group consisted of 55 BAC (-) women. In all participants, BAC was diagnosed using mammography and C-IMT was measured using B-mode and Doppler ultrasonography. BAC was defined as two linear calcification depositions in a conical periphery or as calcific rings at the mammographic evaluation. Doppler spectrum samples were obtained from 2 cm proximal to the main carotid artery bifurcation. Results: Postmenopausal female patients ranging in age from 40 to 86 included in this study. When the groups were adjusted for age, a statistically significant difference was found between mean C-IMT of BAC (+) and BAC (-) groups (0.81 +/- 0.2 vs. 0.69 +/- 0.2 mm; p<0.001). No significant differences were observed between BAC (+) and BAC (-) groups in terms of PSV, EDV, RI. Conclusion: The findings of the present study suggest that BAC, diagnosed by mammography, is independently associated with C-IMT. C-IMT measurement is suggested as a useful tool to detect early atherosclerotic changes. However, haemodynamic variables (PSV, EDV, RI) were not statistically different between the BAC (+) and BAC (-) groups. Prospective larger cohort studies are needed to further elucidate whether BAC is an independent risk factor for cardiovascular disease.

    Case of Tunica Albuginea Cyst Associated with scrotal calculi

    Recently testicular lesions have been seen more frequently because of the widespread using of ultrasonography in urological practice. Cysts related to the testis include tunica albuginea cysts, simple testicular cysts, epidermoid cysts, and cystic rete testes. Cysts arising from the tunica albuginea are relatively rare. Tunica albuginea cysts arise within the capsule that surrounds the testicle. Although cysts of tunica albuginea are usually incidental findings, patients may present with pain, swelling and firm pinhead sized mass. These cysts are thought to be of mesotelial origin. Although they may be mistaken for malignant testicular tumors on physical examination, sonography facilitates the correct diagnosis and unnecessary surgeries. We report a patient who was synchronously diagnosed with scrotal calculi and tunica albuginea cyst.

    Case Report: Spontaneous urinoma: Combined treatment with percutaneous and endoureteral catheterization

    A urinoma is defined as encapsulated collection of chronically extravasated urine. Obstruction of the ureter by calculus, surgical ligature, neoplazms, or obstruction at the bladder outlet, such as by posterior urethral valves, is the most common cause of the urinomas. Spontaneous urinoma during renal colic is rare. Here, we present a case with spontaneous urinoma occured during an attack of renal colic. In addition, treatment strategies of urinomas caused by different etiologies are discussed with the scope of the current literature.

    Colloid cyst mimicking intracranial hemorrhage after head trauma

    Trauma patients consist vast majority of the patients who admit to emergency department, and most of them have a head trauma. A 58-year-old patient was taken to emergency department with head trauma, and a hyperdense lesion neighboring to third ventricle was detected. A diagnosis of colloid cyst was made in the patient who was being followed up for hemorrhage. In patients with head trauma, colloid cyst may easly be confused with intracranial hemorrhage due to hyperdensity. The aim of this report is to emphasize the importance of clinical thinking in the differential diagnosis of hyperdense lesion on computed tomography imaging of a patient with head injury.

    Color Doppler Imaging of the Orbital Vessels in Ankylosing Spondylitis

    Purpose: To analyze the hemodynamic features of orbital blood flow velocities using Doppler ultrasonography in ankylosing spondiylitis (AS) patients, as well as to compare these results with those of healthy controls. Methods: 33 AS patients and 32 healthy controls were consecutively included in the study groups. The same radiologist performed ocular blood flow measurements. Peak systolic velocity (PSV), end diastolic velocity (EDV), and resistive index (RI) were measured in the central retinal artery (CRA), posterior ciliary arteries (PCAs), and ophthalmic artery (OA). Resistive index was used to assess arterial resistance; it was automatically calculated as RI [(PSV-EDV)/PSV]. Results: There were no significant differences in the PSV, EDV, and RI of the OA, CRA, and PCAs between AS patients and controls. Conclusion: This result suggests no possible contributory role of vascular structures in formation of uveitis in AS. We believe that our preliminary results need to be complemented with further studies, particularly including AS patients with uveitis and rheumatic diseases with other ocular involvement.

    Comparison of Conventional Bone Scintigraphy and Whole-Body Magnetic Resonance Imaging for the Detection of Musculoskeletal System Metastases

    Purpose: In this study, the aim is to compare the findings of conventional bone scintigraphyand whole-body magnetic resonance imaging for the known primary malignancy in patientswith bone metastases.Methods: 34 patients were studied with retrospectively and prospectively. The detectionsensitivity, specificity and adaptation of bone metastases were examined whit whole-bodymagnetic resonance imaging and bone scintigraphy.Results: Whole-body magnetic resonance imaging has a higher sensitivity than bonescintigraphy for the detection of metastases.Conclusion: Whole-body magnetic resonance imaging can be used for bone metastases forscreening as an alternative imaging modality

    Complex fracture of the clivus after head trauma

    Clivus fracture (CF), which is usually reported to accompany with head trauma, has high mortality rates. Early diagnosis of CF is rare because of high mortality rates and inadequate urgent radiologic techniques; however, diagnosis rates are increasing with computed tomography images obtained in high resolution and thin sections. In this article, radiologic and clinical features of 2 patients who were detected to have longitudinal CF after head trauma are presented and accompanying pathologies and its importance for prognosis are discussed under the light of literature data composed of a small number of reports.

    The Consideration of Diffusion MR Imaging, Dynamic Contrast-Enhanced MR and T2 Relaxation Time Measurements in Distinguishing of Cholesteatoma with Chronic Otitis Media

    Purpose: The aim of our study is to investigate the contribution of DWI, DCE-MRI, ADC measurements and T2 relaxometry in diagnosis of distinguishing cholesteatoma and COM Materials and Methods: Our study is created by retrospectively reviewing MR images of a digital archive of totally 41 patients that were clinically or surgically diagnosed as COM and cholesteatoma. DWI, ADC values, T2 relaxation time, and CEP of lesions between the two groups were compared statistically. CEP was qualitatively evaluated. ROC analysis and McNemar statistics test were performed. Level of significance was determined as p<0.05. Results: Cholesteatoma detection sensitivity in DWI is determined as 100%, and specificity was determined as 86%. ADC measurements revealed a statistically significant difference in differentiating COM and cholesteatoma (p<0.05). When T2 relaxometry were compared between two groups there was no statistically significant difference (p>0.05). When the groups were assessed according to the CEP, 13 of the COM cases showed type 1 (progressive heterogeneous enhancement), 2 cases did not show enhancement (type 3) and 6 cases showed type 4 (progressive homogeneous enhancement). Nineteen patients with cholesteatoma showed type 2 (rim-like enhancement), 1 case showed type 4. Statistical analysis of CEP revealed significant differences between types 1 and 2 (p<0.001), between types 2 and 3 (p=0.005) and between types 2 and 4 (p<0.001). Conclusion: DWI, ADC values and DCE-MRI may be beneficial in detecting cases of cholesteatoma of a high percentage.
