Öğe The Effectiveness of New Oral Anticoagulants in theTreatment of Lower Extremity Venous Thrombosis: ARetrospective Clinical Study(2021) Düzgün, Ali Cemal; İlkeli, EkinObjective: This clinical study included a retrospective evaluation of 138 patients using new oralanticoagulants (rivaroxaban, dabigatran, apixaban, edoxaban) for lower extremity deep veinthrombosis.Methods: 138 patients using new oral anticoagulant drugs were included in the retrospective study.Demographic characteristics, diagnosis and treatment charts, radiological images, epicrisis notes ofthe patients were recorded. Records of each patient were reviewed during their 12 months of follow- up. New oral anticoagulant efficacy; It was evaluated by measuring thigh and calf diameters beforeand after treatment, whether there was recanalization in Doppler USG (recanalization> 75%),recanalization time, bleeding side effects and recovery times with treatment, and recurrence rates.The data were analyzed statistically using a statistics program.Results: A significant difference was found when the mean thigh diameter measurements measuredat the beginning of the treatment (Mean ± SD: 75,41) and the mean thigh diameter measured afterthe treatment (Mean ± SD: 54,63) were compared (p<0.05). A significant difference was foundwhen the mean calf diameter measurements (Mean ± SD: 43.99) measured at the beginning of thetreatment and the mean calf diameter measured after the treatment were compared (p<0.05). Post- treatment Doppler USG showed complete recanalization rate in 54% (75 patients) at 3 months and25% (35 patients) at 6 months. At the end of the 6th month, the total recanalization rate occurred in80% (110) patients.Conclusions: New oral anticoagulants (rivaroxaban, dabigatran, apixaban, edoxaban) are veryeffective in the treatment of lower extremity deep venous thrombosis, both in providingrecanalization and in terms of clinical improvement in early and midterm.Öğe EVALUATION OF MID-TERM CLINICAL AND ECHOCARDIOGRAPHIC OUTCOMES OF ALFIERI (EDGE-TO-EDGE) MITRAL VALVE REPAIR(Düzce Üniversitesi, 2014) Bollukçu, Ahmet; Çiloğlu, Ufuk; Alibeyoğlu, Şebnem; Dağsalı, Sabri; İlkeli, EkinAim: The present study aimed to evaluate mid-term clinical and echocardiographic outcomesof the Alfieri edge-to-edge repair performed on patients with moderate to severe mitralregurgitation (MR).Methods: Thirty patients (mean age, 61.2±11.2 years; 18 males) with moderate to severe MRof different etiologies and a pulmonary artery pressure (PAP) of gt;40 mmHg who underwent aAlfieri repair between 2005 and 2011 were included. All patients were selected and followed bythe same surgical team and cardiologist. The Alfieri edge-to-edge repair with ring annuloplastywas performed. The patients were postoperatively evaluated in terms of degree of MR, functionalcapacity according to the New York Heart Association (NYHA) classification, PAP, left atrialdiameter (LAD), atrial fibrillation, ejection fraction (EF), risk of functional mitral stenosis,etiological factors, and mortality.Results: Postoperative NYHA class, MR and LAD values of the study patients were significantlybetter as compared to the preoperative values (pÖğe Heyde's syndrome(SAGE Publications Inc., 2014) İlkeli, Ekin; Albeyoğlu, Şebnem; Çiloğlu, Ufuk; Dağsalı, SabriGastrointestinal bleeding due to colonic angiodysplasia can be associated with calcified aortic stenosis. This association is referred to as Heyde's syndrome. Aortic valve replacement can prevent recurrent gastrointestinal bleeding in these cases. We describe the case of a 46-year-old woman with congestive heart failure related to aortic stenosis, and severe anemia, with multiple angiodysplasias on the ileum and colon. After aortic valve replacement, there were no further episodes of bleeding and her hemoglobin levels normalized. © The Author(s) 2013.Öğe Intralesional Epidermal Growth Factor for Diabetic Foot Ulcers(Coll Physicians & Surgeons Pakistan, 2022) İlkeli, Ekin; Demircan, Fethiye Berna Göktaş; Düzgün, Ali Cemal; Arabacı, Hakan; Uysal, Ayhan; Kanko, MuhipObjective: To determine the role of the intralesional recombinant epidermal growth factor (rEGF) in the healing and prevention of extremity amputation in advanced diabetic foot ulcer patients. Study Design: Observational study. Place and Duration of Study: Department of Cardiovascular Surgery, Duzce State Hospital, Duzce, Turkey, between November 2018 and September 2019. Methodology: A total of 58 patients with diabetic foot ulcers that were treated at the study place were enrolled. The lesions were graded with Wagner Classification System. EGF (75 microg of Heberprot-P) vials were stored at +4 degrees C and cold-chain requirements were followed. EGF 5 mL was dissolved with 0.09% saline solution; and 0.5-1 ml of the solution was injected into the tissues and edge of the lesions regularly. The data was evaluated at the end of two years of the treatment period. The primary objective was wound healing, formation of granulation tissue; and the secondary objective was the prevention of lower extremity amputation. Results: Diabetic foot ulcers wound healing was achieved in 93.1% (n=54) of patients with the formation of granulation tissue. The complete recovery was observed in 94.1% (n=32) of the patients who had Grade III and IV lesions. Lower extremity amputation was performed in two (3.4%) subjects. The lesions of two patients required flap surgery. The most common adverse events were tremor and syncope. Conclusion: Recombinant epidermal growth factor is highly effective for the treatment of diabetic foot ulcers and prevention of extremity amputation. Objectives: The impact and results of pharmacomechanical thrombectomy treatment on patients with deep vein thrombosis in the symptomatic acute and subacute phases were assessed. Patients and Methods: Pharmacomechanical thrombectomy treatment was applied in a single session for 38 patients with lower extremity deep vein thrombosis in the acute and subacute phases between May 2012 and June 2014. Venography was performed and each assessment was made based on lysis rates and clinical results. Results: Deep vein thrombosis was found in the left lower extremity in 25 patients (65%) and in the right lower extremity in 13 patients (35%). No patient was found to have bilateral deep vein thrombosis. Thrombus localization was in the iliofemoral area in 17 patients (44%), the iliocaval area in three patients (8%), and the femoropopliteal area in 18 patients (56%). When thrombus resolution was assessed at the end of the process, grade III and grade II lysis was achieved in 36 (94%) out of 38 patients. Complete resolution was achieved in 28 patients (74%) in the acute and subacute groups. Conclusion: Pharmacomechanical thrombectomy provides very satisfactory results in a single procedure, as a new method in the treatment of acute and subacute deep vein thrombosis.Öğe Right Ventricular Perforation during Pericardiocentesis: A Case Report(Düzce Üniversitesi, 2015) İlkeli, Ekin; Çapcı, Selim; Koç, Ayşegül; Ekinözü, İsmailPercutaneous drainage of pericardial effusion occurring due to Cardiac or non-cardiac causes is a common practice. Bu yazıda; acil servise karın ağrısı, hipotansif atak ve nefes darlığı ile başvuran 60 yaşında bir kadın olgu sunuldu. Bilgisayarlı tomografi ve arteriyografi sonrası distal splenik arter hizasında rüptüre anevrizma tespit edildi. Batında yaygın sıvısı, sol hemitoraksta plevral sıvısı olan hipotansif hastaya, acil cerrahi müdahale ile anevrizma rezeksiyonusplenektomi uygulandı.
